medical expenses Grants for Severely Disabled

This system subsidizes a portion of insurance medical treatment when a person with severe disabilities who is a member of the employee health insurance receives a medical institution due to illness or injury.
Expenses that are not covered by insurance medical treatment, such as differential beds during hospitalization, and meals are not subsidized.
If you have a mental illness, you will only be able to visit the hospital.
For those who are eligible, we will issue Medical Certificate.

• Those who have been issued a first- or second grade certificate of the physically disabled
• Those whose intelligence index is judged to be 35 or less
• Those who have been issued a 3rd grade certificate of disability and whose intelligence index is judged to be 50 or less
• Those who have been issued a first-class mentally handicapped health and welfare notebook

In order to prevent the spread of COVID-19, the validity period of Medical Certificate with severe disabilities will be as follows for those who will be valid for Medical Certificate with severe disabilities from March 31, 2020 to February 28, 2021.
Please note that the handling differs depending on the reason for acquisition.

The validity period will be extended for one year.
Medical Certificate, whose validity period has been extended, will be sent by mail before the expiration date.

[Those whose intelligence index is judged to be 35 or less]

When a notebook of love (nursing notebook) is issued, please update the Medical Certificate for severely disabled people as usual.

If the Mental Handicapped Health and Welfare Handbook is updated, please update the Medical Certificate for severely disabled people as usual.

By presenting your health insurance card and Medical Certificate with severe disabilities to the window of a medical institution, you can receive medical treatment without having to pay for it.
If you do not have a health insurance card, you will not be able to use Medical Certificate.

In order to receive the medical expenses subsidy for severely disabled people, please bring the following "Required for Application" and apply to the ward office Insurance and Pension Division National Health Insurance Section.

If you are unable to present a Medical Certificate with severe disability due to unavoidable reasons, or if you go to a hospital that does not handle this system or a hospital outside the prefecture, you can get a refund if you go through the procedure at the ward office Insurance and Pension Division National Health Insurance Section.
If you are a member of the medical system for elder senior citizens, please submit "Transfer Account Designated registration form". If you visit a hospital outside the prefecture and pay a partial contribution, or if you pay a partial contribution using other medical expenses subsidies such as a specific disease in the prefecture, pay the designated account approximately 6 months later.
However, please note that you will need to apply at the ward office Insurance and Pension Division if you pay a portion of the co-payment without presenting the Medical Certificate for a severely disabled person in the prefecture.

• Deposit passbook of the financial institution to which the bank transfer
• If there is a high medical costs paid by the health insurance union for the month of medical treatment to be applied, or a payment notice that allows you to confirm the amount of additional benefits.

※In addition, when transferring to an account other than the applicant, seal, the delegate (applicant) is required.

If high medical costs is paid by employee health insurance, the amount will be paid excluding that amount.
Please note that the statute of limitations will expire on the 1st of the month following the date of your consultation, and you will not be able to apply.

In the following cases, a report is required.
・When the grade of the disability certificate changes
・When the employee health insurance you subscribe to changes
・When Address changes

※If you move out of the city, you will not be able to use Medical Certificate, so be sure to notify us.
※In the unlikely event that you consult a medical institution using Medical Certificate after you lose your eligibility due to transfer, etc., you will be required to refund the amount you received. Please contact the Health and Social Welfare Bureau Medical Aid Division for procedures.
