Grants for severely disabled people medical expenses

What is the medical expenses Grant for Severely Disabled Persons?

This system subsidizes the co-payment of insurance medical treatment for people with severe disabilities who are enrolled in employee health insurance when they go to a medical institution due to illness or injury.
Non-insurance benefits such as hospitalized beds, document fees, physical checkup, selected medical expenses, etc., and meals will not be subsidized.
Grants for those with mental illness are only available to the hospital.
For those who are eligible, we will issue a Medical Certificate with severe disabilities.

Those who are eligible

  • Those who have been issued a certificate of the physically disabled for grades 1 and 2
  • Those whose intelligence index is determined to be 35 or less
  • Those who have been issued a 3rd grade certificate of the physically disabled and whose intelligence index is judged to be 50 or less
  • Those who have been issued the 1st grade Mental Handicapped Health and Welfare Handbook

When you go to a doctor

By presenting your health insurance card (conventional health insurance card, minor insurance card or qualification confirmation) and Medical Certificate with severe disabilities to the window of a medical institution, you can receive medical treatment without paying yourself.
Without a health insurance card (conventional health insurance card, minor insurance card or qualification confirmation), Medical Certificate with severe disabilities cannot be used.

Method of application

In order to receive the medical expenses Grant for Severely Disabled Persons, please bring the following "Required for Application" and apply to the ward office Insurance and Pension Division National Health Insurance Section.

What is required for application

  • Medical Certificate grant application for severely disabled people (PDF: 108KB)(Please read the notes on the back and fill in the thick frame.)
  • Handbook for the Physically Handicapped, Handbook for Love, Handbook for the Mentally Handicapped, etc.
  • Confirmation of the contents of employee health insurance (conventional health insurance card, qualification confirmation, notification of qualification information, printed on the insurance card information screen of the minor portal, etc.)

When the severely disabled Medical Certificate cannot be used

If you are unable to present Medical Certificate with severe disabilities due to unavoidable reasons, or if you go to a hospital that does not cover this system or a hospital outside the prefecture, you can get a refund if you complete the procedures at Insurance and Pension Division National Health Insurance Section, ward office in your ward.
If you are a member of the medical system for elder senior citizens, please submit "Transfer Account Designated registration form". If you visit a hospital outside the prefecture and pay a partial co-payment, or if you pay a partial co-payment using other medical expenses subsidies such as specific diseases, you will be transferred to your designated account approximately 6 months later.
However, please note that if you are unable to present the severely disabled Medical Certificate in the prefecture and pay a partial contribution, you will need to apply at the ward office Insurance and Pension Division.

Necessary for procedures

  • medical expenses Payment Application Form (PDF: 84KB)
  • Confirmation of the contents of employee health insurance (conventional health insurance card, qualification confirmation, notification of qualification information, printed on the insurance card information screen of the minor portal, etc.)
  • Severely disabled Medical Certificate
  • medical expenses receipt (with patient name, total number of insurance medical treatment, period of medical treatment, amount of receipt, medical institution name)
  • Bankbook of the financial institution to which the transfer is made
  • If there is a high medical costs provided by the health insurance union for the medical treatment month to be applied or a payment notice that allows you to confirm the amount of additional benefits, etc.

※When transferring funds to an account other than the applicant, the delegate (applicant) seal is required.

Please be careful

If high medical costs is paid by employee health insurance, the amount will be paid excluding that amount.
Please note that the statute of limitations will expire five years from the 1st of the month following the day of the consultation, and you will not be able to apply.

Other Notifications

In the following cases, notification is required.
・When the grade of the Handbook for Persons with Disabilities changes
・When employee health insurance has changed
・When Address changed, etc.

・Confirmation of the contents of employee health insurance (conventional health insurance card, qualification confirmation, notification of qualification information, printed on the insurance card information screen of the minor portal, etc.)

※If you move out of the city, you will not be able to use Medical Certificate, so please be sure to notify us.
※If you go to a medical institution using Medical Certificate after you lose your eligibility due to transfer, etc., you will be required to refund the amount you received. Please contact the Health and Social Welfare Bureau Medical Aid Division for procedures.
