Please notify us of the change (return to Medical Certificate for infant).
Notification is also possible by mail. Please send the following "Required for Application" to your ward office Insurance and Pension Division National Health Insurance Section Payment Section.
In particular, the window will be very crowded during the moving season, so please use the notification by mail when moving from Yokohama to another municipality.

・Original Medical Certificate for infant

Immediately after the date of transfer

※Please do not forget to return the product immediately after the transfer date.
※The validity period of Medical Certificate for infant in Yokohama is until the day before the transfer date. Please note that after the date of transfer, you will lose your eligibility and will not be eligible for Subsidies for Medical Expenses for Pediatric Treatment from Yokohama City.
※If you go to a medical institution using Medical Certificate for infant after you lose your eligibility, you will be required to refund the amount of the grant. Please contact the Health and Social Welfare Bureau Medical Aid Division for procedures.
