・Medical Certificate for infant Application Form (and Consent Form) Transfer of Childregistration form
・Your child's Medical Certificate for infant (in the case of mail, a copy is enclosed)
・Your child's health insurance card (in the case of mail, a copy is enclosed)
・Parents' private seal (using vermilion)
※For private seal, it is necessary to submit a report when using Medical Certificate due to traffic accidents.
