Please prepare required documents and apply for a refund.

What is required for application

■What you need
・Child medical expenses Application Form
・Medical Certificate for infant (in the case of mailing, a copy is enclosed)
・Your child's health insurance card (in the case of mail, a copy is enclosed)
・Receipt (patient name, total number of insurance medical treatment, medical treatment period, receipt amount, medical institution name)
※If you submit the original receipt to the health insurance union, you can also copy it.
・Bankbook or cash card of the transfer destination financial institution (in the case of mailing, enclose a copy of the place where the transfer destination is known)
■Required only for those who apply
・private seal (using vermilion)
※When transferring funds to an account other than the applicant (guardian), the delegate (applicant) seal is required on the application form.
・Medical expenses and high medical costs paid by employee health insurance as the medical treatment month to be applied, and payment notices that can confirm the amount of additional benefits, etc.
※This is necessary if the health insurance union you have provided medical expenses (70% or 80% of medical expenses's health insurance burden), high medical costs, and additional benefits. It is not necessary if there is no payment.
・Doctor's instructions (a copy (copy) is also acceptable if the original is submitted to the health insurance union)
※Necessary when making treatment equipment such as pediatric amblyopia treatment glasses and corsets at the direction of a doctor.
