I have lost my health insurance card for the medical system for elder senior citizens, so I would like to apply for a reissue.

Please apply for re-grant at your ward office, Insurance and Pension Division National Health Insurance Section.
What is required for application
・Identity Confirmation Documents
(1)Please bring your name, date of birth, or Address printed with the expiration date.
(2)If possible, please bring a certificate with a photo attached and a document issued by a public office or a similar organization.
・Driver's license, driving history certificate
・My Number Card
・Long-term care insurance card
Etc.
・If an adult guardian has been appointed, a copy of the certificate of entry, etc.

・As a general rule, it will be delivered by mail and it will take a few days to arrive. If you plan to receive a medical examination immediately, you can issue a “medical certificate” as a substitute for your health insurance card, so please apply to the ward office Insurance and Pension Division National Health Insurance Section.
・If you lose your insurance card due to theft, etc., please send it to the police.
