A funeral fee of 50,000 yen will be paid to those who performed the funeral (mourner) for those who were insured in Kanagawa Prefecture Elderly Medical Care.

・Documents that confirm the name of the mourner, the name of the deceased, and the date of the funeral (funeral receipt, funeral letter, etc.)

・Mourner private seal (using vermilion) ※Necessary when specifying an account in a name other than the mourner as the transfer destination.

・Deposit passbook (those specified as the transfer account)

・Insurance card of the deceased insured (elderly medical care insured card) (unnecessary if already returned)

※After two years from the day after the funeral, the statute of limitations will expire and you will not be able to apply.
