How to fill in the back of "Voting Guide"

Please fill in [A] "Invoice (and Oath Form)" on the back of the "Voting Information" and bring it to the early polling place. The reception is done early.
Also, if you do not have the "Voting Information", please fill out the form (invoice and oath) provided at the early polling place.
●Person who performs absentee vote in Election Administration Committee of stays (outside Yokohama-shi)
[A] Fill out the “Invoice (and Oath Form)” and [A] “Voting Paper Destination” and mail it to the Election Commission of your ward in your Address area (electoral list registration place) (FAX or E-mail is not allowed).
