About medical care to carry out in elementary and junior high schools

In Yokohama City, as a “medical care support project at municipal schools,” nurses are dispatched to municipal elementary and junior high schools where children who need medical care on a daily basis are enrolled, and provide necessary medical care. In dispatching nurses, we provide medical care and support in cooperation with schools, family doctor, and related organizations so that elementary school student students can take care of themselves.

Eligible elementary school student students

elementary school student students who fall under the following 1 and 2

  1. Among elementary school student students enrolled in elementary schools, junior high schools, compulsory education schools and special needs schools (excluding special needs schools with school nurses) who have been diagnosed by family doctor as being able to go to school or require medical care on a daily basis.
  2. Persons who are deemed to need assistance from a professional point of view at the Medical Care Implementation Coordination Meeting composed of doctors and school officials

※ elementary school student students are not eligible for medical care.


Contents of targeted medical care

"Medical care" refers to medical activities such as suction and injection performed using medical equipment, not general medical care in general. Nurses perform at hospitals, etc., but in daily life, they and their families do it. Care that can be carried out on your own will be carried out at school under the supervision of teachers.

Care targeted for nurse dispatch

  • Sputum aspiration (in the oral cavity, in the nasal cavity, inside the trachea canonale)
  • Oral nutrition (nasal, stomach wax, intestinal wax)
  • Urinalysis (including self-control urination requiring technical guidance from nurses)
  • Other medical care that we deem necessary

Care not eligible for nurse dispatch

  • Care (insulin self-injection, blood glucose measurement, self-injection, etc.) without the need for technical guidance from nurses.
  • Care that can be done by non-nurses (stoma care for colostomy, regular medications of therapeutic drugs, etc.)
  • Non-daily medical care (required treatment only in case of emergency or deterioration, specific seasonal or short-term care, and advanced medical nursing skills)

Implementation site

  • Municipal Elementary Schools, Junior High School, compulsory education School and Special Needs School (excluding special schools for the physically handicapped) where elementary school student students are enrolled.

※ It covers medical care performed on the school premises on the school day (Monday to Friday). Activities that take place on weekends and holidays, school holidays, activities outside the school premises, and accommodation events are not eligible.


Implementation method

  • According to the time of medical care, the medical association of the school district entrusted by Yokohama City will coordinate and dispatch nurses from local home nursing stations to the school.
  • As we perform discussion in conduct coordination meeting held by the Board of Education and coordination with home-visit nursing stations and determine the time and number of nurses dispatch, we may not be able to carry out as desired by parents and schools.

Characteristics of this project

  • This is a business in which a visiting nurse come to school, makes use of breaks, etc., and provides necessary medical care for children.
  • We will provide support for children to work on their own, such as sputum aspiration and urination of sputum, and aim for future independence in the future. If tube feeding or other nurse care is needed continuously, we will provide care for the necessary period of time.
  • We provide care and technical guidance according to the child's medical condition and characteristics so that learning and activities at school can be expanded.
  • Several times a year, through meetings on support by the person, guardian, school, nurse, etc., we will check the status of the person's efforts, how to support the child, and their roles.

Prospects of medical care

  • There are also care, such as aspiration of sputum and urine guidance that enables the child to do alone as they grow up. Nurses will help you acquire skills and support independence so that your child can be reliably done alone to expand your child's social life. We will support the independence of care as your child grows up.
  • Some people do not need medical care due to surgery or treatment.
  • If it becomes independent, the dispatch of nurses will end and care will be carried out safely under the supervision of the school.

※ Some children do not use this business and have been taking care of themselves from the beginning.


To make effective care

  • If you can adjust the number and content of care and medications performed in the school as little as possible, care in the school will be smooth and the impact on school activities will be reduced. Consider as much as possible, such as providing care before going to school and adjusting the time of medicine.
  • It is important to share annual goals and work together so that families, schools, and nurses can promote care based on a common understanding.
  • Let's help your child work on what they can do. Let's continue what you can do at home, including during long holidays.
  • Since the medical condition and condition of the child during the growth period change, you must submit a medical certificate and instructions from family doctor once a year.
  • Schools, nurses, boards of education, etc. check the information, set support goals, and provide appropriate care.

Application for use

  • Please consult with the school you plan to enroll or the school you are currently enrolled in. Application for use is made through the school.
  • If you are a new student, please do not wait for physical checkup when you go to school.
  • At the time of application, along with the submission of family doctor's medical certificate and home-visit nursing instructions, we will ask you about the status (contents, number of times, etc.) of medical care currently being performed at home, the school you are attending, or community nursing center. You. Please check with the school for required documents and the timing of submission.
  • We will consider school activities first and adjust the schedule for medical care, so we may not be able to do so as to parents wish.
  • The dispatch of nurses will be coordinated by the City Medical Association. (The dispatch may be made from multiple offices.)
  • Application for use is made every year. (Applications in the middle of the year is acceptable.)
  • From the application for use to the start of use, there is a period to examine the application documents and make a contract procedure. During that time, parents will be required to provide care.
  • If care skills become independent or care is no longer needed by treatment, the dispatch of nurses will end. If you leave school for a long period of time due to hospitalization, we will stop dispatching nurses.
