About work certificate to submit to Yokohama-shi (2024)

We will guide you through how to fill out the form when an employee is asked to create a "work certificate" to be used for entering a nursery school in 2024.

When creating a work certificate to be submitted to Yokohama City, be sure to check this page before making it.

◆ Be sure to check the description example before filling in.
◆ Yokohama City may contact you regarding the contents of the description.
◆ The submitted certificate cannot be returned, so if there are any omissions, please submit it again through the applicant.
◆ When making corrections, please erase them with double lines. Correction fluids and disappearing ballpoint pens cannot be used.

Special attention should be paid to the description

Employment (planned) Period, etc. (No.3)

 ○For fixed-term employment, please indicate the first contract start date.
 ○If you are planning to start working as soon as you enter a nursery school and cannot indicate the employment start date, please indicate so in the “Remarks column (No. 14)”.

Working hours (No.6)

 ○Please indicate the number of working days and working hours based on the employment contract, not actual results.
 ○For the number of working days per month, please indicate the number of working days from the first day to the last day of the month.
 ○If the number of days and hours have been changed due to the use of the reduced working hours system for childcare, indicate the number of days and hours before the change are described, and “For childcare”.
  Please indicate the working hours and period after the change in "Whether or not to use the reduced working hours system (No.12)." 

Date of returning to work (No.11)

 ○If it is possible to shorten childcare leave when you are offered a job at a nursery school, etc., please indicate the expected date of return to work after the shortening.
  ※ If you start using the service on April 1, you must complete the childcare leave by the end of April, the month in which you start using the service on April 1, and return to work by May 1.           

Remarks column (No.14)

 ○If any of the following applies, or if there are any other items that you want to describe, please write it in the remarks column.
 ・If you plan to increase working hours and days (including cases where it can be increased when a nursery school decides to use it), please describe the following three points.
  (1) Time to increase working hours, days, etc.
  (2) The number of working days per week or per month if it can be increased
  (3) Working hours per day and per month if it can be increased
  [Method of description] From the month when the use of nursery schools, etc., it will be increased to 8 hours a day (128 hours a month).
 ・If you have already retired or have decided on Retirement, be sure to include the date of Retirement (scheduled) date.
 ・If you have taken maternity leave or childcare leave more than once, please indicate the period.
  [Method of description] Prenatal and postpartum leave: Reiwa ○ Month ○ day to Reiwa ○ month ○ day
 ・If you are working alone (including your schedule), please indicate the period of your assignment. If the end date of assignment is undecided, only the start date will be stated.

  [Method of description] Single transfer period: Reiwa ○ Month ○ day to Reiwa ○ month ○ day
 ・Even if the working hours of “Working Time (No. 6)” are not working at night, it is usually done at least twice a month, such as night shifts or business trips involving accommodation (excluding overtime).
  If you have any night work, please indicate so.
  ※Nighttime work refers to work including 24:00. Therefore, if the working hours of “Working Time (No. 6)” extends beyond 24:00 pm
   We assume that there are two or more night work at least twice a month.
 ・In the case of a fixed-term employment contract, please indicate whether or not the contract has been renewed (including planned) after the expiration of the current contract.

To their parents

◆ The proofs of the submitted documents may be checked in the "Contact Information".
◆ Additional documents may be required to check your employment status.
◆ If you are a self-employed person, you may be asked to submit objective materials related to employment.
  Example: Business license, business registration report, copy of registry, final income tax return, contract contract with business partner, copy of business consignment contract, etc.
◆ In the case of irregular employment, a shift table etc. may be required.
◆ If you apply for a sibling at the same time, please copy and submit the number of siblings.
◆ In the case of a new application or relocation application, please submit the one listed within 6 months from the submission date in principle. The date of entry before that date
  In this case, it may be disadvantageous in terms of benefit authorization and adjustment of usage. ※This is not the case to confirm the current situation. 
◆ Please be sure to write the name of elementary school student in the "Parents" column.
◆ If you have any other questions, please contact the ward office Children and Families Support Division.
