1)Being not a person listed in Article 3, Paragraph 1 of the Yokohama City Contract Regulations (March 1964, Yokohama City Regulations No. 59) and being a person who has the qualifications stipulated by the provisions of Paragraph 2 of the same Article.
(2)A company registered in "Other consignment, etc.: Sterilization" in the list of qualified persons in the Yokohama City General Competitive Bidding Qualified Person List (related to goods, consignment, etc.).
(3)In accordance with the instructions of the school, the designated type and quantity of equipment can be delivered by the day before the health check-up date. In addition, if health check-up equipment is urgently needed on the day of implementation of the health check-up, additional delivery shall be possible in principle.
(4)Sterilization is possible at the trustee's own facility.
(5)Being a person who has not received the nomination suspension measures based on the measures outline such as Yokohama City nomination suspension on any day from the bid participation intention application deadline to the bidding date.
