From March 1, 2024, attachment of a family register certificate is no longer required when submitting a family register notification!

The first and spouse of family register (if one of the first or spouse is a foreigner or dead, the other person will be applicant)

The form can be obtained in the following way.

• Page (outside site) of Yokohama-shi electronic application, report serviceDownload (When printing, be sure to check the notes on the download page.)
• It is distributed at the Family Registry Division of each ward office in the city.
• You can also use paper from other municipalities.

From March 1, 2024, attachment of a family register certificate will no longer be required when submitting a family register notification. However, in the case of family register, which is not computerized, the attachment of family register cannot be omitted. Therefore, except in the case of transfer of domicile within the same ward, obtain certificate of family register at your permanent address and attach it to the notification form.

Please report to any of the following ward offices (municipalities).

• Location (Address)
• Current domicile
• New permanent address

From Monday to Friday, 8:45 a.m. to 5:00 p.m.
From 9:00 a.m. to noon on the 2nd and 4th Saturday

Outside of reception hours at the Family Registry Division window

During this time, applications will be accepted at night and on holidays, but since the contents cannot be confirmed on the spot, it will be treated as custody. On the next opening day, the staff in charge of family register checks the contents, and if there is no defect, it will be accepted as far back as the date of acceptance, and the date of submission of the transfer of domicile notification will be the "Notification Date" stated in family register.

If you are transfer of domicile notification that requires inquiries to other municipalities or other organizations, and cannot be confirmed because the contact information is closed, it will be treated as custody. We will make an inquiry again on the next opening day, and if confirmation is confirmed, the date of submission of the transfer of domicile notification will be the "Notification Date" stated in family register.
