Person who reports and can do
Relatives and cohabitants
notification period
Within 7 days (including that day) from the day when the fact of death was learned (if there was a death abroad, within 3 months from the day when the fact was learned)
※If the seventh day falls on the closing day of the ward office, the next opening day will be the last day of the notification period.

Reception desk
Please report to one of the following ward offices (municipalities)
- Dead land
- Home address of the person who died
- Location of applicant (Address)
Each ward office Family Registry Division
From 8:45 a.m. to 5:00 p.m. Monday through Friday
From 9:00 a.m. to noon on the 2nd and 4th Saturday
※Excludes public holidays and New Year holidays
About ward office window opening agency of the second, fourth Saturday
Reception desks for each ward office night and holiday
Outside reception hours at the Family Registry Division window, such as at night or on holidays
Notes
During this time, the contents cannot be confirmed immediately after the reception, so it will be treated as custody. On or after the next opening date, family register officials will check the contents and if there are no defects, they will be accepted back to the date of acceptance and the date of submission of death registration/death notification form will be the "date of notification" stated in family register.
About fire burial permission application
To perform cremation, please submit a fire burial permit application together with death registration/death notification form.
※If cremation has been completed, death registration/death notification form has been notified.

Condolence window (Tsurumi Ward and Seya Ward only)
Using the Tsurumi Ward Office and Seya Ward Office as model districts, we have set up a "condolence window" to guide necessary procedures and help create applications.
Those who can use it are the bereaved families of those who had registered as residents in Tsurumi Ward and Seya Ward when they died.

Notes
In death registration/death notification form, which requires inquiries to other municipalities or other institutions, if the contact information is closed and cannot be confirmed, it will be treated as custody. The date of submission of the death registration/death notification form will be the “Notification Date” stated in family register.
