Relatives and cohabitants
notification period
Within 7 days (including that day) from the date of knowing the fact of death (if there was death outside the country, within 3 months from the date on which the fact was learned)
※If the seventh day falls on the closing date of the ward office, the next open day will be the last day of notification period.

Reception desk
Please notify one of the following ward offices (municipalities):
- Land of death
- Home address of the deceased person
- Location of applicant (Address)
Each ward office Family Registry Division
From Monday to Friday, 8:45 a.m. to 5:00 p.m.
From 9:00 a.m. to noon on the 2nd and 4th Saturday
※Excludes public holidays and New Year's holidays

Precautions
During this time, the contents cannot be confirmed immediately after reception, so it will be treated as custody. After the next opening date, the staff in charge of family register will check the contents, and if there is no defect, it will be accepted as far back as the date of submission of death registration/death notification form will be the "Notification Date" stated in family register.
About fire burial permission application
To perform cremation, please submit a fire burial permission application together with death registration/death notification form.
※If cremation has been completed, death registration/death notification form has been notified.

Precautions
In death registration/death notification form, which requires inquiries to other municipalities or other organizations, and cannot be confirmed because the contact information is closed, it will be treated as custody. We will make an inquiry again on the next opening day, and if confirmation is confirmed, the date of submission of death registration/death notification form will be the "Notification Date" stated in family register.
