About condolence window (we support procedure when imminent person died)

About procedure in ward office of person who died, we perform trial setting of "condolence window" in model ward office to plan burden reduction of bereaved family.
At the condolence window, we will check the necessary procedures based on the information we asked at the time of advance reservation, and on the day of the next agency, we will help you create an application at the condolence window and create a list of necessary/required procedures We will create and guide you to each section window.
If you do not know what kind of administrative services the deceased had received, or you do not know what procedure to start, we will guide you at the condolence window.
The ward office that has a congratulatory window
・Tsurumi Ward Office (in the ward hall on the first floor)
・Seya Ward Ward Office (next to Family Registry Division on the second floor)
※You can use the bereaved families of those who had registered as residents in Tsurumi Ward and Seya Ward when they died.
Please note that other wards are not available.

How to use it

Reservations start from 8:45 on Monday, January 22, 2024.
①[Web Reservation] Or make a reservation by phone.
◆Please make a reservation within a few days after submitting death registration/death notification form. (approximately 2 to 10 business days)
◆You can make a reservation until 16:00, 5 business days before your desired date.
◆When making a reservation, we will ask you for the name, date of birth, Address, date of death, etc. of the person who died.

※We recommend that you make a reservation on the web, as it may not be possible to leave immediately during customer support.
②We will call you from the condolence window the day before or two days before the reservation date.
We will tell you about the necessary belongings by phone.
③Please come to the condolence window on the reservation date.
At the condolence window, we will assist you in creating an application and guide you to the window.
Please note that you will not be able to apply for all procedures at the condolence window.
Also, depending on the content, the procedure may not be completed at once.
Reservation slots (4 slots per day)
①From 9:00 to 9:50
②From 10:30 to 11:20
③From 13:30 to 14:20
④From 15:00 to 15:50
All of them are on weekdays only. (Opening days on the 2nd and 4th Saturday are not allowed.)
