About condolence window (we support procedure when imminent person dies)

About procedure in ward office of person who died, we perform trial setting of "condolence window" in model ward office to plan burden reduction of burden of bereaved family.
At the condolence window, we will check the necessary procedures based on the information you asked at the time of advance reservation, and on the day of the next agency, we will help you create an application form at the condolence window and make a list of necessary/required procedures We make and guide to each section window.
We will guide you at the condolence window if you do not know what kind of administrative services the deceased had received, or do not know what procedure to start.
A ward office with a condolence window
・Tsurumi Ward Office (in the ward hall on the first floor)
・Seya Ward Ward Office (2nd floor next to Family Registry Division)
※It is available to the bereaved families of those who have registered as residents of Tsurumi Ward and Seya Ward when they died.
Other wards are not available, so please forgive me.

How to use it

Reservation starts at 8:45 on Monday, January 22, 2024.

◆Please make a reservation a few days after you submit your death registration/death notification form. (Approximately 2 to 10 business days)
◆You can make a reservation until 16:00, 5 business days before your desired date.
◆When making a reservation, we will ask you for the name, date of birth, Address, and date of death.

※We recommend that you make a reservation on the web, as it may not be possible to leave immediately during customer service.
②We will call you from your condolences on the day before or two days before your reservation.
We will tell you about necessary belongings by telephone.
③Please come to the reception desk on the day of your reservation.
At the condolence window, we will assist you in creating an application form and guide you to the counter.
Please note that you will not be able to apply for all procedures at your condolences.
Also, depending on the content, the procedure may not be completed at once.
Reservation slots (4 frames per day)
①9:00 to 9:50
②10:30 to 11:20
③13:30 to 14:20
④15:00 to 15:50
Both are available on weekdays only. (Opening date on the 2nd and 4th Saturday is not allowed.)
