Notification of transfer by mail of person who does not have my number card, Basic Resident Register card (moving out of the city)

It is procedure of mail when person who does not have my number card, Basic Resident Register card moves from Yokohama city to municipalities outside the city.

  • The person himself / herself
  • Head of household or person and member of same household
  • proxy/agent (a person who has a power of attorney from the person or a person in the same household)

Precautions

Even though Address is the same, those who have different households need a power of attorney.

Prior to moving or within 14 days from the date of moving

    Duration of the procedure

    After receiving the notification at the ward office Family Registry Division, usually about 10 days before the transfer certificate arrives.
    ※Before moving, please allow enough time to complete the procedure.

    Please be sure to provide a contact information that can be reached during the day. (If the contact information listed in the notification is different from the daytime contact information, please indicate each.)

    2.A copy of the identity verification document of the person to be notified

    • Driver's license
    • Health insurance card, etc.

    Please fill in the reply address ("Previous Address" or "Address") and attach a stamp.

    4.(In the case of notification by voluntary proxy/agent) Power of Attorney

    Procedure Flow

    1. required documents to the ward office of Address (Yokohama)Family Registry Division before moving.
    2. Mail a transfer certificate (documents required for transfer at the municipal office after moving) from the ward Family Registry Division
    3. Bring your required documents, including the transfer certificate, and submit a transfer notification at the municipal office where you are moving.

    Precautions

    • If the transfer notification processing has not been completed at the ward office Family Registry Division before moving, you will not be able to submit a transfer notice at the municipal office where you move.
    • If you submit a transfer notice by mail, it will take a few days for the transfer at the ward office Family Registry Division. Please submit your report with plenty of time.
    • If the transfer is canceled, you will need to cancel the transfer notice at the ward office in your ward.
