Testing medical institutions will conduct PCR tests and list all necessary items in the “test application form” (name of the test medical institution, name of the test conductor, test sample) in the “test application form”. Regardless of whether it is positive or negative, the test results will be communicated to the prospective person (or family, etc.) by telephone or the like from the test medical institution.
5. Those who are planning to enter will report the test results to the facility they plan to enter.
6. Testing medical institutions, etc. will attach the “Inspection Application Form” to the “PCR Test Results Report and Consignment Fee Bill”.
7. Yokohama City will pay the commission fee through the designated financial institution based on the invoice.
