Status report of COVID-19 infection, etc. (for entrance facilities, etc.)

When the following infection situation occurs in facility after May 8, 2023, I would like report to public health center and Motoichi jurisdiction section. Depending on the content of the report, the health center will provide guidance on infection control as appropriate.
　・Situation that needs to be reported to public health centers (Health and Welfare Center each ward) (if any of 1 to 3 applies)
　① When two or more fatalities or serious patients suspected to be due to COVID-19 infection occur within one week
　② When more than 10 people or more than half of all users are infected.
　③ ①Even if it does not fall under 2 and 2, it is suspected that outbreaks exceed normal outbreak trends, especially when the facility manager deems it necessary to report.

　・Please put the Health and Social Welfare Bureau Elderly Health and Welfare Department corona outbreak report e-mail address (kf-corona@city.yokohama.jp) in the CC when reporting e-mail to the public health center.
