※For the purpose of the survey and how to respond, please refer to “02_ Request for Survey Cooperation (explanatory materials for business establishments)”.

All designated home care support establishments operating as of August 1, 2022
※However, establishments that have been suspended, abolished, or relocated outside the city after August 1, 2022 are not eligible for the survey.
