※A part of the examination is performed by the trustee "Kanagawa welfare service promotion society" of trustee. Therefore, please be aware that "Kanagawa Welfare Service Promotion Association" may contact the person in charge of the establishment for confirmation.

Establishments that wish to receive stickers and posters (including those that apply for grants) should enter from the following form.

From Thursday, July 9, 2020 to Monday, August 31, 2020

After August 1, mail will be sent to the offices in order from the offices where the examination has been completed.
※The work of stickers and posters will be carried out by the "Kanagawa Welfare Service Promotion Association".

☑ It will be invalid if you fill out the check sheet on the attached sheet directly and submit it by e-mail or paper. Please be sure to fill in the form from the electronic form. (The same applies when applying for a grant.)
☑ If at least one of the items on the check sheet, except for "5" is checked, stickers and posters cannot be distributed.
☑ Even if there is an item that is not currently available in each item on the check sheet, if you can work on it within two weeks after entry, check "I have done (make countermeasures)" Please check and enter.
☑ Establishments that apply for a subsidy for the “Nursing Care Service Operating Expenses Subsidy Project” are required to enter, and if at least one of the items is checked except for “5” is checked, Grants are not available. (There is no sticker or poster for short-term entrance life care and short-term nursing care nursing care.)
☑ Be sure to write down the "arrival number" and "inquiry number" issued after entry in the electronic application form. If you cannot confirm the number, you have not entered it, so please enter again. It is not possible to modify the contents after entry. If modifications are required, please re-entry again.
☑ The expiration date of the sticker for this entry is until March 31, 2021.
☑ We will not conduct on-site confirmation, so please take measures at the responsibility of the business site.
