　I would like to express my sincere gratitude to each office for your daily efforts to prevent the spread of infectious diseases, including COVID-19.
　Although the state of emergency has been lifted, new lifestyles require continued thorough infection prevention measures.
　In particular, at day-care service establishments, due to the characteristics of multiple users and staff members meeting together to provide services, there is an environment that tends to be “three dense (closed, dense, close)”, and there is a high risk of cluster occurrence, further measures are needed.
　Therefore, we have summarized the measures to prevent infection that Yokohama City considers.
　First, please check the specific infection prevention measures on the status of infection prevention measures check sheet (hereinafter referred to as "check sheet").

　By posting stickers and posters and disseminating the details of the efforts, users can receive the service with peace of mind, and staff at the office can respond while checking the measures.

※Limited to establishments located in Yokohama City.

※The content of the poster also changes with gold and silver.
※Of the examination items, establishments that fall under 1 are "Silver", and establishments that fall under 2 are "Gold". (Only one of them)
※Part of the examination will be performed by the "Kanagawa Welfare Service Promotion Association" of the trustee. For this reason, please note that the Kanagawa Welfare Service Promotion Association may contact the person in charge of the establishment for confirmation.

Businesses wishing to receive stickers and posters (including those applying for grants) should enter from the following form.

From Thursday, July 9, 2020 to Monday, August 31, 2020

After August 1, mail will be sent to the establishments sequentially from the establishments that have been reviewed.
※The work of sending stickers and posters will be performed by the commissioned "Kanagawa Welfare Service Promotion Association".

☑ If you fill out the check sheet on the attached sheet directly and submit it by e-mail or paper medium, it will be invalid. Please be sure to fill in the application form. (The same applies when applying for a grant.)
☑ If at least one of the items on the check sheet is checked except for "5", stickers and posters cannot be distributed.
☑ Even if there is an item that is not currently available among the items on the check sheet, if you can work within two weeks after entry, check "It is made (measures are being taken)" Please enter and enter.
☑ Entry is required for establishments that apply for subsidies for the “Nursing Service Operating Expenses Subsidy Project”, and if at least one of the items is checked except for “5”, Grants cannot be received. (There is no sticker or poster for short-term entrance life care and short-term entrance care care.)
☑ Be sure to write down the "arrival number" and "inquiry number" issued after entry in the electronic application form. If the number cannot be confirmed, the entry has not been completed, so please enter again. You cannot modify the contents after entry. If you need to modify it, please re-enter it.
☑ The expiration date of the sticker accompanying this entry is until March 31, 2021.
☑ We do not perform on-site confirmation, so please take measures at the responsibility of the establishment.
