It will be issued to establishments that fall under the following (1) to (4).
However, establishments newly designated from February to June 2020 have an exception for (4). (See “How to calculate the amount of grants” below.)

Establishments located in the city that have been designated as offices as of the end of June 2020, and will continue to provide services while continuing to take three-crow measures.

Implement the contents of the “Check Sheet for Infection Prevention Measures” (application form [attached sheet]) and meet the standards.
If there are items that do not fall under items 2 to 4 (not completed), you will not be able to receive a grant.

-The following contents are only applicable to day-care service establishments ~

※ Establishments that meet the standards will receive a subsidy, as well as a sticker of the “Infectious Disease Countermeasures Office” and an A4 size poster describing the details of the efforts (sent sequentially from August).

<Notes>

・The benefit results used by Motoichi for calculation are each amount after the examination decision provided by the Federation of National Health Insurance to Motoichi. Please note that the monthly remuneration paid to the business office does not include the user's burden and the amount is different.
・About payment results of month when we performed error adjustment, reclaim, there may be difference of payment results and amount used by Motoichi for calculation. Thank you for your understanding.

② Determination of the amount of grant
Calculation base amount Determine the amount of grant according to the amount of grant.
The amount of grants is divided into 11 stages as follows.
The grant amount (total) is the grant amount for three months (the amount multiplied by 3) of the grant amount per month in the table below.
※Calculation base amount If the amount is less than 100,000 yen, it will not be issued.

Because it is difficult for establishments that have received new designation to calculate the amount of subsidies based on past benefits results, based on the benefits results of Motoichi establishments, the amount of subsidies shown in the table below will be used.
