About COVID-19 infectious person correspondence allowance assistance business for Yokohama-shi care service establishment worker (commonly known as infected person correspondence allowance business)
Overview
In order to be able to continue to provide the necessary services to home recuperators due to COVID-19 infection, Yokohama City provides employees who have provided services directly to home recuperators by nursing care service establishments. Response allowance (hereinafter referred to as "care allowance") If "is provided, a subsidy equivalent to the allowance will be issued.

※Due to the 2021 subsidy project, benefits paid after April 1, 2022 are not eligible for this subsidy.
Please be sure to complete the payment of benefits to workers by the deadline.
Subsidized establishments
(1) Visiting service establishments (* 1) that provide visit services to COVID-19 positive patients who are undergoing home medical treatment.
(2) Outpatient service establishments (* 2) that provide visiting services to those who are positive for COVID-19 during home medical treatment by staff of the establishment.
※1 Visiting service offices
Home-visit nursing care establishments, home-visit nursing establishments, home-visit rehabilitation establishments, regular patrol / ad-hoc home-visit nursing establishments, night-time home-visit nursing establishments, small-scale multifunctional home-care establishments and nursing small-scale multifunctional home-care establishments (limited to visiting services), home care support establishments and home care management guidance establishments
※2 service offices
Day care establishments, community-based day care establishments, nursing home care establishments, dementia-responsive day care establishments, tsusho rehabilitation establishments, small multifunctional home care establishments and nursing small-scale multifunctional home care establishments (limited to commuting services)
※3 Includes visiting and outpatient service establishments for preventive care services and comprehensive care prevention and daily life support business (designated services and preventive care management).
Subsidized allowance
※Please note that you cannot apply in duplicate with expenses recorded in other supporting projects. .
<Infected person response allowance>
This is an allowance to be paid on a daily basis to workers who provide services directly in the same space for positive persons during home medical treatment.
A. Amount
"5,000 yen" per day.
・ If a single positive person is provided multiple times by the same employee on the same day, 5,000 yen will be paid per day regardless of the number of times.
・ If the service is provided by two people with another worker, each worker will be paid.
・ It will be paid for each positive person who has provided the service.
B. Number of days
Until the recuperation period of the positive person is deemed to have expired, the number of days in which services have been provided directly in the same space.
However, since the medical treatment period is expected to be 10 days in principle, if it is considered that medical treatment is required for more than 10 days, individual consultation with the city is required.

Notes
・Benefits can be calculated from the date of onset (certified by the public health center) until the end of the medical treatment period.
It will be the day when the visit service to your home was provided (from April 1, 2021).

※Please use your own signature for "Receipt Confirmation Signature".
② A copy of documents that can be confirmed by the employee during the period of providing the service directly to "users infected with COVID-19 infection at business sites" entered in the first form in the same space.
(Example: Service Provision Record, etc.)
③ A copy of documents that confirm that the target person has been paid the allowance (eg, salary statement, etc.)
※ If it is added to the corporation's own allowance or other allowance, please make a breakdown so that you can see the amount paid as an infected person's allowance.

※ Please prepare and submit one application document for each business establishment and each positive person.
※ As a general rule, it is an application once for each positive person who provided the service, but if you have to apply for additional applications, please contact the following inquiries.
