Seal Registration (document inquiry method)

It will take a few days to register, so please complete the procedure with plenty of time.
Please bring the following two items to the counter.

1. A document confirming your name (such as a health insurance card)
2. Registered private seal

※An inquiry will be sent to your home in the evening of the day you receive it. Upon receipt of the documents, fill out the required items, seal, and bring it to the ward office within 30 days. You must fill out the form yourself, but you can also delegate the second procedure to proxy/agent.

※If you are currently in Seal Registration, please bring your private seal registration card (card) and private seal. However, you don't have to worry about it.

※If proxy/agent does the procedure, the required documents will be different. Please start again from the first question.
