Notification of loss of private seal registration card (for example, when the card is lost)

If you lose your private seal registration card (card) that you gave at the time of Seal Registration, or if you cannot identify the private seal registration card (card), you will need to submit a lost private seal registration certificate. If you do this, Seal Registration will be abolished.

What you need

The required items vary depending on the person who completes the procedure at the counter.

When the person (registered person) performs the procedure

  • private seal registration card (card) ※Only when the registration number is no longer identified
  • Items that can be used to verify your identity (My Number Card, Driver's License, Health Insurance Card, etc.)
  • Application for change of Seal Registration (i.e., issuance of certificates, loss, abolition, loss of private seal)Page (outside site) of Yokohama-shi electronic application, report servicePlease print out the application form, or obtain it at the ward office window.)

Precautions

In order to obtain Seal Registration Certificate at the same time as the loss of private seal registration certificate, a new application for Seal Registration is required along with the loss of private seal registration certificate. Please refer to the Seal Registration application for required documents and procedures.

When proxy/agent performs procedures

  • Identification of proxy/agent (My Number Card, Driver's License, Health Insurance Card, etc.)

Precautions

  • If the person coming to the counter is proxy/agent, the "document inquiry method" will be used. We abolish current registration when we have you bring answer book sent from ward office at a later date. ※Depending on the postal circumstances, it may take a few days (about one week) for the procedure to be completed.
  • If the Seal Registration Certificate is acquired at the same time as the loss report of the loss of the private seal registration certificate, it will be newly added together with the lost private seal registration certificate.Seal Registration ApplicationI need it.
