Notification of loss of private seal registration card (for example, when the card is lost)

If you lose your private seal registration card (card) that you have given at the time of Seal Registration, or if you can no longer identify the registration number of your private seal registration card (card), you will need to report the loss of your private seal registration card. With this procedure, Seal Registration will be abolished.

Notes

If you wish to obtain Seal Registration Certificate at the same time as your private seal registration card loss report, you will need to apply for a new Seal Registration along with your private seal registration card loss report. Please refer to the Seal Registration application for details on required documents and procedures.

Notes

If the person coming to the counter is proxy/agent, the "document inquiry method" will be used. We abolish current registration when we have you bring answer book sent from ward office later. ※Depending on postal circumstances, it may take a few days (about one week) to complete the procedure.
