Seal Registration (proxy/agent document inquiry method)

Registration takes a few days (2 to 3 days if there is no holiday), so please complete the procedure in advance.

If you are in proxy/agent, please bring the following four items to the counter.

1. Power of Attorney created by the person (how to write a power of attorney (PDF: 116KB))
2. Documents confirming proxy/agent's name (driver's license, health insurance card, etc.)
3. Registered private seal
4. proxy/agent's approval

※An inquiry will be sent to your home in the evening of the day you receive it. When you receive the documents, fill out and seal required items and bring it to the ward office within 30 days. You must fill out the documents yourself, but you can also delegate the procedures at the ward office to proxy/agent again.

※If you are currently in Seal Registration, please bring your private seal registration card (card) and private seal. However, you don't have to worry about it.
