Deduction of medical expenses
The medical expenses Notice can be used to declare medical expenses deductions. The medical expenses Notice can be used as a attached documents in the "Details of medical expenses Deductions" (from 2018). However, if there are items that are not listed in the notice of medical expenses or the names of medical institutions other than Kanagawa Prefecture and Tokyo will not be printed. Therefore, it is necessary to prepare a “medical expenses Deduction Statement” based on a separate receipt and attach the statement to the declaration form. (In this case, medical expenses receipts must be kept for 5 years from the deadline for final tax return.)
In addition, although the amount equivalent to the copayment amount stated in the notice may differ from the amount of the receipt, it may be calculated based on the amount stated in the notice.

Declaration of self-medication tax system
The self-medication tax system (exception of medical expenses deduction) requires that individuals who take “certain measures” as an effort to maintain and improve health and prevent illness from switching OTC drugs (needs guidance drugs) from January 1, 2017. When you purchase a drug that has been diverted from medical use), you can receive an income deduction for the purchase cost.

In order to declare the application of the self-medication tax system, you will need "documents that clarify that certain measures have been taken".
Those who have consulted Yokohama City Health Checkups can receive a certificate.
Fill in the claimant's name, fill in the seal, and submit it to issue a certificate. If you would like to issue a certificate, please contact your ward office, Insurance and Pension Division National Health Insurance Section.
