Announcement of Health Checkups and medical expenses of Yokohama

The medical expenses Notice can be used to declare medical expenses deductions. The medical expenses Notice can be used as a attached documents in the "Details of medical expenses Deductions" (from 2018). However, if there are items that are not listed in the notice of medical expenses or the names of medical institutions other than Kanagawa Prefecture and Tokyo will not be printed. Therefore, it is necessary to prepare a “medical expenses Deduction Statement” based on a separate receipt and attach the statement to the declaration form.
