About procedure of medical system for elder senior citizens by mail

The following procedures for the medical system for elder senior citizens can be notified and applied by mail. Please note that if there are any deficiencies, we may contact you or return it.

Qualifications

Application for re-issuance of identification card for medical care for the elderly (insurance card)

Fill out required items on the application form for reissuing a certificate of insured medical care for seniors and send it to the ward office Insurance and Pension Division National Health Insurance Section, the ward office of your ward. We will mail you the issued insured person's card.

Application for grant of limit application certificate and limit application / standard burden reduction certificate (reduction certificate)

After confirming that it is eligible for grant, we will mail the application form, so please contact the ward office Insurance and Pension Division National Health Insurance Section.

Other applications for qualification

For other eligibility applications, please contact your ward office Insurance and Pension Division National Health Insurance Section.

Benefits

When applying by mail, it is necessary to print out the application form (see table below) distributed at the counter by yourself.
In the case of application by mail, we may contact you to confirm the contents. In addition, it may take more time to transfer than usual.

  • Application forms, etc. sent by the Kanagawa Medical Association for the Elderly
  • Copy of identification card for late-elderly medical care insured person (insurance card)
  • A copy of the account confirmation document

Application for payment of large sum total nursing care expenses
  • Application forms, etc. sent by the Kanagawa Medical Association for the Elderly
  • Copy of identification card for late-elderly medical care insured person (insurance card)

※If you have applied before, you need to apply for each calculation period.

Application for payment of medical expenses
(In case of emergency or other unavoidable circumstances, such as sudden illness
If you have a medical examination without a health insurance card)

※Please contact your ward office Insurance and Pension Division National Health Insurance Section in advance, as it may be necessary to add attached documents depending on the content of your application.

Treatment not covered by insurance in Japan are not eligible.

※For attached documents, please see the Overseas Medical Expenses Information Flyer (PDF: 112KB). Please fill in the shaded part of the application form.

※Please fill out the medical treatment statement in A3 size on both sides.

※A copy of the passport (passport) must be accompanied by the applicant's name and the record of entry and exit pertaining to the application are known.

Application for payment of funeral expenses

※If you do not have any confirmation of the mourner and the funeral day, please contact the ward office Insurance and Pension Division National Health Insurance Section. It may be necessary to attach a funeral expenses payment application form.

Applications by heirs, adult guardians, etc.

If an heir or adult guardian makes an application, you may be asked to add attached documents. When an heir or adult guardian applies, please contact the ward office Insurance and Pension Division National Health Insurance Section of the ward where the heir or adult ward resides.
