The current insurance card will be abolished on December 2, 2024, and medical institutions will use a minor insurance card in principle.
This time, Yokohama National Health Insurance (hereinafter referred to as "National Health Insurance") We will inform you of the details associated with the abolition of the insurance card in the Kanagawa Medical System for Elderly Seniors (hereinafter referred to as the “late term”).
The basic items are as described in "My Number Card and Health Insurance Card will be integrated."
In the future, we will update this announcement as appropriate and inform you of the information.
Terminology
・The "current insurance card" issued before December 2 is later referred to as an "insurance card".
・My number card that has been registered for the use of an insurance card is later called a "minor insurance card".

Notices

From August 1, 2025 onward

The National Health Insurance and the latter term insurance card expires on July 31, 2025, but since the insurance card cannot be issued, a minor insurance card will be issued.
A "qualification confirmation" will be issued to those who have not been registered.
We will issue a "Notification of Qualification Information" to those who have registered.
Both the National Health Insurance and the second semester will be sent in a push type (no procedure is not required) and mailed by July 2025.

About National Health Insurance limit application, standard burden reduction authorization

With the introduction of the online qualification confirmation system at medical institutions, etc., it is possible to confirm the limit application category with the consent of the individual, so it is not necessary to present a limit application certificate. (You do not need to apply for renewal in August every year.)

※Those who fall under the following need to apply for grants such as a limit application certificate.

・In the case of medical institutions that do not have an online qualification confirmation system

・Municipal tax-exempt households who have been hospitalized for more than 90 days in 12 months before the application date will receive further reductions in dietary allowances

[About the medical system for elder senior citizens]

The above contents are the same for the second half of the year.

However, please note that the following points will be different from the National Health Insurance in the second half.

・Issuance of the limit application certificate, etc. will end with the abolition of the insurance card on December 2.

※As with the insurance card, the issued limit application certificate, etc. can be used until the expiration date stated.

・The “burden category” after December 2 will be described in the “Qualification Confirmation Form” based on the application.

※In the past, those who have applied for a limit application certificate, etc., those who are eligible for the application, we will list the burden category in the “Qualification Confirmation Form” sent in July next year.

Other information related to facilities

In the country, "persons who have not obtained a My Number Card" or "Persons who have a My Number Card but have not registered for use of a health insurance card", etc. We assume as a target, but in addition to this.
"When it is difficult for a consultation with a minor insurance card, such as the need for a third party, such as a caregiver, to accompany the person requiring special attention and assist in qualification confirmation."
It is a target person who can issue a “qualification confirmation” upon application.
It seems that the content is very relevant to the facilities, but the details of this response are under consideration, so we will inform you on this page once the details are decided.
