From December 2, 2024 (Reiwa 6), my number card and health insurance card have been integrated.
If you have various medical expenses grant certificates (such as Medical Certificate for infant, Medical Certificate with Severely Disabled Persons, Medical Certificate Single-parent Homes, etc.), please present them to medical institutions as before.
If you have any questions about the transfer of a minor insurance card, please contact the call center.

We have established a dedicated call center for the transfer of minor insurance cards for insured persons in the Yokohama City National Health Insurance and the medical system for elder senior citizens.
If you have any questions about your minor insurance card, such as how to consult after December 2, please contact us here.
In addition, the last 4 digits of the personal number (My Number) sent to the Yokohama City National Health Insurance Insured Person ※For more information, please contact us.
※With the integration of My Number Card and Health Insurance Card, the national notice will be issued so that everyone can use My Number Card as an insured person's card with peace of mind.
Based on this, we send a notification with the last four digits of the personal number (My Number) to all insured persons of Yokohama City National Health Insurance.

How long can I use my paper insurance card?

In the case of Yokohama City National Health Insurance

Available until July 31, 2025.

Attention

However, in the following cases, it will be up to the respective deadline.

  • Those who celebrate their 75th birthday before July 31, 2025: Until the day before your birthday
  • Those who celebrate their 70th birthday before July 31, 2025: Until the end of the birthday (in the case of 1st, the end of the previous month)

    Medical care for the elderly in Kanagawa Prefecture

    Available until July 31, 2025.
    The paper insurance card (light blue) currently issued can be used until the expiration date (July 31 of next year (2025)).

    Attention

    • Even after your health insurance card has been abolished, please bring it without discarding it until it expires.

    Even after the transition to the minor insurance card, the paper insurance card can be used until the expiration date.

    If the paper insurance card is abolished (from December 2, 2024)

    After the abolition of the paper insurance card, if you join the employee health insurance (*), or if you re-issue it due to a change or loss of the contents of the paper insurance card (burden ratio, etc.) after the abolition date, you will receive a “Notification of Qualification” or “Confirmation Form” depending on whether or not you have registered your insurance card.
    In addition, about insured person of medical system for elder senior citizens, we hand qualification confirmation regardless of use registration of minor insurance card only until July 31, 2025 by provisional measures of country (If lost, application is required.)

      Those who have registered for use of a minor insurance card

      We issue "news of qualification information" (the medical system for elder senior citizens is scheduled to be issued in August 2025).
      We will send it to those who have a minor insurance card so that the registered insurance information (name, date of birth, insured number, partial burden ratio, etc.) can be easily confirmed.

      Those who have not registered for use of a minor insurance card

      "Qualification Confirmation Form" will be issued.
      You can receive insurance medical treatment by presenting it to a medical institution.

      Notes and Supplements

      • We will send a “Qualification Confirmation Form” or “Notice of Qualification Information” to all insured persons during July of next year (2025).
      • The size of the “Qualification Confirmation Form” is scheduled to be the same size as a paper insurance card in both the National Health Insurance and the second half.
      • You can receive a "Notice of Qualification Information" by presenting it with your My Number Card when you visit a medical institution where a minor insurance card cannot be used.
      • It is not possible to consult a medical institution just by "Notice of qualification information".

      About cancellation of use registration of minor insurance card

      You can cancel the registration of your minor insurance card by applying to the employee health insurance where you are enrolled.

      ・Documents without a photo issued by a public office, such as a health insurance card, a long-term care insurance card, or a pension notebook.

      (Note) If you move to another medical insurer before confirming the cancellation of registration after applying for cancellation of use registration, the medical insurer, etc. after the transfer will be notified to the medical insurer, etc. that you have previously joined Please state that you have applied for cancellation.

      For those who have a minor insurance card but need a qualification confirmation

      In principle, those who have a minor insurance card will not be issued a qualification confirmation, but those who meet the following requirements can receive a qualification confirmation by application. If you have a valid insured person's card, you will not be issued a qualification confirmation even if you apply during the expiration date.

      FAQs

      Q1 I have already registered for the use of my number card insurance card. When I am 75 years old and move to the medical system for elder senior citizens, do I need to apply for a new insurance card for my number card?

      A. Once you have applied for your My Number Card insurance card, you do not need to repeat the procedure.

      Q2 Is it okay to carry my number card with you?

      A. Even if it can be used as a health insurance card, information with high privacy, such as consultation history and drug information, will not enter the IC chip of the card. If you drop or lose it, you can use the toll-free number 24 hours a day, 365 days a year.

      Q3 Can I have a medical examination without a My Number card?

      A. We send "qualification confirmation" to person who does not have my number card, person who does not apply for insurance card of my number card. By presenting the “Qualification Confirmation Form” to medical institutions, etc., you can still receive insurance medical treatment.

      Q4 Can I get a medical examination at hospitals where my number card is not available?

      A. If you have an insured person's card or a “qualification confirmation (same size as the current insurance card)”, you can receive insurance medical treatment by presenting it to a medical institution. If you are using your My Number Card insurance card, you will be issued a "Notice of Qualification Information (A4 size)" that allows you to easily check your own qualification information. By presenting, you can get insurance medical treatment.
