About special collection of National Health Insurance premium from pension

What is special collection?

The special collection means that the head of household pays the National Health Insurance premium by fund transfer or payment slip (normal collection), but deducts the National Health Insurance premium from the public pension received by the head of household in advance. It is a mechanism to pay.
Already, special collections from the public pension, which is paid every two months, has been implemented in Long-term Care Insurance Premium and others, but due to the revision of the national system, the National Health Insurance premium will also be subject to special collection from the pension, which has been implemented since October 2016.

Households subject to special collection

In principle, households that meet all of the following requirements (1) to (4) are subject to special collection from pension. (The base date is April 1st.)
(1)The head of the household has National Health Insurance, and all insured persons in the household are between 65 and 74 years old.
(2)The head of the household receives a public pension subject to special collection of 180,000 yen or more per year.
(3)The head of household, Long-term Care Insurance Premium, is specially collected from the public pension.
(4)The sum of the National Health Insurance premium and Long-term Care Insurance Premium for that year does not exceed one half of the amount received by the public pension (*).
 ※If you receive more than one pension, you will be deducted from the highest priority pension as specified by a Cabinet Order. (Even if you are an old age Employee Pension Plan (Kosei Nenkin), the public pension below will take precedence).

In the special collection of National Health Insurance premiums, Long-term Care Insurance Premium is deducted from the same pension as the public pension, which special collections.

When a new special collection is made

From June to September of the year in which special collection starts, ordinary collection (*) as before, in October, December and February of the following year, the amount excluding the total amount of ordinary collection from the annual insurance premium Please divide into three times and pay by special collection.
In April, June and August after the following year, please pay the same amount as the payment amount in February of the previous year by special collection (this is called "provisional collection"). In October, December and February of the following year, the amount excluding the total provisional collection amount from the annual insurance premium will be divided into three times and paid by special collection (main collection).

About "national health insurance premium amount decision letter of advice" of the year to become special collection newly

The National Health Insurance premium will be determined in June, but households that will be specially collected from October will be judged in July according to the requirements of laws and regulations.
Therefore, in the “National Health Insurance Premium Decision Notice” sent in mid-June informs all heads of household as “normal collection” (“fund transfer” or “payment notice”). Households subject to special collection will send a "National Health Insurance Premium Notice" again in mid-July and will inform you that special collection will be made from October.
In addition, we do not notify in July about household which is targeted for special collection and normal collection continues.

(We will inform you of the special collection from October. From June to September, payment will be made by normal collection. )

Regardless of special collection, if you wish to pay by fund transfer.

For households that meet the requirements for special collection, special collection from pension is a general rule, but if you wish to pay by fund transfer, you can change the payment method. In that case, you will need to do so.

  • The total amount paid does not differ depending on the payment method. Also, due to laws and regulations, payment by payment slip cannot be selected.
  • If you have not paid your National Health Insurance premium, you may not be able to accept your application.
