How to reissue your My Number Card if you cannot change your Address or name, etc.

If there is a change in the expiration date of your period of stay due to Address, name, or foreign national, you will need to change the information on your My Number Card.

When Address or name of my number card cannot be changed

　　(1) When there is no space to be listed in the My Number Card entry column
　　(2) If your My Number Card has expired
　　(3) In the case of a My Number Card that has expired due to departure
　　(4) When moving in from outside Yokohama city, there is no description of the immediately preceding suburban Address
　　(5) When more than 90 days have passed since the date of notification of transfer from outside Yokohama city
　　(6) Lost, damaged, or IC chip is poor

In the case of the above, the Address or name of My Number Card cannot be changed.
We will mail the application form for reissuing my number card, so please check the following and request by email.

The application form will be sent to Address on resident certificate where you want to reissue your My Number Card.
The application form will be shipped after the next business day. Please apply with plenty of time.

When you receive the application form, please apply by smartphone or mail.
After creating your My Number Card, the ward office will send you a letter of advice (postcard) by post-free mail, so please make a reservation and come to the agency to receive it.
Your My Number Card will be collected when you hand your new My Number Card, so please be careful not to dispose of it.
