How to reissue your My Number Card if you cannot change your Address or name, etc.

If there is a change in the expiration date of your period of stay due to Address, name, or foreign national, you will need to change the information on your My Number Card.

When Address or name of my number card cannot be changed


  (1) When there is no space to be listed in the My Number Card entry column
  (2) If your My Number Card has expired
  (3) In the case of a My Number Card that has expired due to departure
  (4) When moving in from outside Yokohama city, there is no description of the immediately preceding suburban Address
  (5) When more than 90 days have passed since the date of notification of transfer from outside Yokohama city
  (6) Lost, damaged, or IC chip is poor

In the case of the above, the Address or name of My Number Card cannot be changed.
We will mail the application form for reissuing my number card, so please check the following and request by email.

How to receive a reissue application form by mail (application by e-mail)

Reason for the need to reissue my number card
Free of charge

(1) Address, name, etc.
(2) Because the expiration date has expired
(3) Expired by moving out of the country

Paid (1,000 yen)

(4) Due to transfer from outside Yokohama city, there was no mention of the immediate Address
(5) Due to the transfer from outside of Yokohama city, the procedure deadline has exceeded
(6) Because my number card was lost or damaged
(7) Because the period of stay was extended, but the extension procedure was not performed within the expiration date of the My Number Card (for foreign nationals only)

Applicable Reason for Reissue and

Please write the following name and address in the e-mail and send it to the application e-mail address.

Name of person who wants to reissue my number card, Address of resident certificate, date of birth, Phone number
(If an agent sends an email) The name, relationship with the person, Phone number

  • The application form will be sent to Address on resident certificate where you want to reissue your My Number Card.
  • The application form will be shipped after the next business day. Please apply with plenty of time.
  • If you have lost your My Number Card, please.If you lose your My Number CardPlease check it out.
  • When you receive the application form, please apply by smartphone or mail.
  • After creating your My Number Card, the ward office will send you a letter of advice (postcard) by post-free mail, so please make a reservation and come to the agency to receive it.
  • Your My Number Card will be collected when you hand your new My Number Card, so please be careful not to dispose of it.
