・The person or family applies for "certification for long-term care" at the ward office Elderly and Disabled Support Division. We can have Local Elderly Care Management Center (community care plazas), home care support companies, etc. act on your behalf. （Click here for a list of community care plazas）

(Renewal application)
・Even if you are already certified as requiring long-term care, you will need to apply for renewal before the end of the validity period to continue using The Long-term Care Insurance services. Renewal applications can be made 60 days before the expiration of the certification validity period.

・The Long-term Care Insurance (needs nursing care and support required) certification application (PDF: 368KB) (Click here for entry example (PDF: 497KB))
・The Long-term Care Insurance Insured Card (issued at the age of 65)
・The name of your family's medical institution, doctor, etc.
・Medical insurance card (for those aged 40 to 64 only)

• The Long-term Care Insurance (needs nursing care and support required) certification
• The Long-term Care Insurance insured person's card (original)

　※When submitting the application form, please attach the The Long-term Care Insurance insured person's card (original). Applications can be accepted even if they cannot be attached due to loss or other reasons.

About presentation method, presentation by window of ward office or mail is possible.

If you fall under the Address Land Special Exception System (Note 1), please check the "Insurer Number" on the The Long-term Care Insurance Insured Card and submit it to Elderly and Disabled Support Division in each ward.

Note 1) The Address Special Exception System is a system in which an insured person in The Long-term Care Insurance enters a long-term care insurance facility or a paid nursing home in another municipality, and transfers resident certificate to the location of the facility.

• When care manager such as a home care support company performs home (care prevention) service plan (care plan), care prevention care management, or when report contents such as the care plan are changed, notification is required Required.
