Application-related: About purchase of specific welfare equipment

1.Payment of purchase of welfare equipment

When an insured person who has nursing care / support certification purchases specific welfare equipment from a specific welfare equipment sales company to use it at home, a part of the welfare equipment purchase cost will be refunded .


※If you live in a nursing home with nursing care or a group home, you cannot use it in principle.

※If you have not purchased from a company designated by the prefectural governor (Yokohama City for businesses in Yokohama City) as a "specific welfare equipment sales company", you will not be able to refund it.

1.Sitting toilet seat

Limited to those that fall under any of the following.
・It is placed on a Japanese style toilet and converted into a sitting style.
・Place on a Western-style toilet to compensate for height
・Electric or spring type that has a function that can assist when standing up from the toilet seat
・Toilet that consists of toilet seats, buckets, etc. and can be moved.
(Limited to those that can be used in the living room.)

2.Replaceable parts for automatic excretion treatment equipment

Replaceable parts of "automatic excretion treatment equipment", a lending event for welfare equipment in The Long-term Care Insurance.
3.Excretion prediction support equipmentIt detects the condition in the bladder and estimates the amount of urine, and notifies the home care recipient or the person who cares for the opportunity of urination.
4.Bathing aid equipment

Equipment for the purpose of assisting in bathing such as maintaining a sitting position, entering and exiting a bathtub, etc., and is limited to any of the following.
・Bathing Chair
・Handrail for bathtub
・Chair in the bathtub
・Bathing table (a table used over the edge of the bathtub, for entering and exiting the bathtub)
・Bathroom
・In the bathtub
・Bathing assistance belt

5.Simple bathtubAir-type or folding type that can be easily moved and does not require construction for water intake or drainage.
6.Part of the suspension of the moving liftThe Long-term Care Insurance's “Mobile Lift,” a rental item for welfare equipment.
7.Slope (*)Limited to those that eliminate steps and do not require construction for installation.
8.Walker (*)It has a function to supplement the walking function of a person who has difficulty walking, and has a structure that supports weight when moving, has four legs and can be held and moved with the upper limbs.
9. Walking aid (*)Limited to Canadian clutch, Loftland clutch, platform clutch and multi-point cane.

(※) 7, 8, and 9 have been added to the target event from April 1, 2024. (For 7.8.9, you can select either "Lending" or "Purchase". Please consult with a care manager or a designated company for welfare equipment. )
   Person considering the purchase of 7.8.9, please confirm Motoichi Q & A other than notice of country.

3.Amount paid

90% of the purchase price (80% or 70% if there is a certain level of income)


※There is a refund limit (annual) as follows:

・When the burden ratio is 10% → 90,000 yen

・When the burden ratio is 20% → 80,000 yen

・When the burden ratio is 30% → 70,000 yen

※As a general rule, items purchased and paid in the past or those of the same type cannot be purchased in duplicate.

4.Procedure

※Yokohama City does not provide in-kind benefits for the purchase of welfare equipment by application and receipt commission payment.

(2)Reception desk

Insurance and Pension Division, the ward office in the ward of Address on the insured card of The Long-term Care Insurance.

(3)Required documents

・Application form for payment of The Long-term Care Insurance benefit expenses (Excel: 23KB) Entry example (PDF: 169KB)

・Receipt

・Those who understand why welfare equipment is necessary (entry in the application form, reason book (reference style) (Excel: 18KB), home service plan, welfare equipment sales plan)

・Pamphlets of purchased welfare equipment (documents that outline the welfare equipment)

※In addition to the above, the following two are required when applying for benefits for "excretion prediction support equipment". Please consult with the designated company of the supplier for preparation of documents.

(1) Confirmation document of medical findings
  ・Opinion Statement of family doctor in Nursing Care Certification Examination
  ・Doctor's findings at service personnel meetings, etc.
  ・Physician's findings in home service plans, etc. heard by care manager and others
  ・Medical certificates, etc. of doctors acquired individually

(2) Decretion prediction support equipment confirmation record style (PDF: 54KB)
  (It is necessary to fill in the form by the sales company of the purchaser.)
