If any of the following applies, please notify the Elderly and Disabled Support Division of each ward.

• When care manager, such as a home care support company, prepares a care plan and care prevention care management.
• When the content of the notification of the establishment that creates the care plan is changed
• When the establishment where the care plan is created is changed (Note 1)

(Note 1) Notification is also required when the degree of nursing care required is changed to support required ⇔ nursing care required.

※Since the seal column has been abolished (February 12, 2021), seal is unnecessary.

• Home and Care Prevention Service Plan Creation and Care Prevention Care Management Request registration form
• The Long-term Care Insurance Insured Card (original)

※Please be sure to submit it by the end of the month. (Excluding Saturdays, Sundays, and holidays and from December 29 to 31)
　After the deadline, payment may be delayed by more than one month.
※Please do not forget to attach the original The Long-term Care Insurance insured card when submitting your home notification, just like the The Long-term Care Insurance application form.
