Report related to home and care prevention service plan creation

When we correspond to any of the following, please report to each ward Elderly and Disabled Support Division.

• When care manager, a home care support provider, etc., performs care plan and preventive care management
• When the notification content of the establishment that creates the care plan are changed
• When the establishment where the care plan is created is changed (Note 1)

(Note 1) Notification is also required when the degree of nursing care is changed to support required ⇔ nursing care.

※The seal column has been abolished (February 12, 2021), so no seal is required.

• Requestsregistration form
• The Long-term Care Insurance insured person's card (original)

※Please be sure to submit it by the end of the first month. (Excluding weekends and holidays and from December 29 to 31)
　Payment may be delayed for more than one month after the deadline.
※As with the The Long-term Care Insurance certification application form, do not forget to attach the original The Long-term Care Insurance insured person's card when submitting a home notification.
