What is The Long-term Care Insurance?

Our society is now aging at an unprecedented rate. In the future, it is expected that the number of elderly people who are bedridden or need nursing care will increase rapidly, and the idea of `` what to do if they need nursing care '' is not only for the elderly and their families, but also for everyone. It is anxious to have.
In addition, the period during which nursing care is required has been prolonged, and the aging of the family members who care is progressing, making it difficult to respond adequately with family care.
The long-term care insurance system was created with the aim of supporting the nursing care, which is the biggest cause of anxiety in retirement, throughout society.
Management of Long-Term Care Insurance System
Yokohama City is the insurer and operates the system.
The long-term care insurance system was implemented in April 2000.
Yokohama City, an insurer, performs duties such as collection of insurance premiums, certification for long-term care, and insurance payments, and operates the system.
In addition, the system was revised in April 2006 to respond to the aging of the population after the establishment of the system.
About notification
If you are a citizen over the age of 65, or if you are a medical insurance member between the ages of 40 and 64 and have a The Long-term Care Insurance insured card, please notify us in the following cases:
- When moving in from another municipality or moving out to another municipality
- When Address, name, household, etc. change
- When you lose your The Long-term Care Insurance insured card or get dirty
- When the insured person dies
- When you enter (enter) a long-term care insurance facility outside the city and move to Address
- When receiving or no longer receiving social security, etc.

Flow from new application to service use
1.Consultation and application
The person or his / her family applies for "certification for long-term care" at the ward office Elderly and Disabled Support Division. Required documents are as follows:
- Application for certification for nursing care and support required (at the counter)
- Long-term care insurance card (issued at the age of 65)
- Those who know the name of your medical institution, doctor, etc.
- If you are a "second insured person" who has medical insurance between the ages of 40 and 64, you must have a health insurance card for your medical insurance.

2.Certification Survey
Investigators will visit their homes, etc., after contacting the ward office or contractor in advance, and conduct interviews with themselves and their families. The survey items are 74 basic surveys and general surveys that are common throughout Japan.
Investigators will be care manager (care managers) belonging to ward staff and offices.
3.family doctor Statement of Opinion
The family doctor specified at the time of application will create a written opinion of family doctor. family doctor describes the status of diseases and other diseases that cause mental and physical disorders.
In principle, requests for family doctor's opinion will be mailed from the ward office to medical institutions.
4.Certification for long-term care
Based on the results of the accreditation survey and the family doctor opinion, committee for certification of need, which is made up of specialists in health, medical and welfare, examines and determines how much care is needed.
Based on the examination and judgment of the committee for certification of need, the ward will certify the degree of care required.
5.Consultation with home care support offices, etc.
<People who have been certified as requiring nursing care>
We talk with care manager of home care support establishment about what kind of service is necessary and ask for creation of care plan. After confirming the care plan, contract with the service establishment and start using the nursing care service.
<Persons who have been certified as needing support>
What kind of service is needed to consult with the Local Elderly Care Management Center (community care plaza, etc.) and care managers of home care support establishments that have been designated as care prevention support, and request the creation of a care prevention care plan. After confirming the care plan, contract with a service establishment and start using services for preventive care.

About user burden
If you use The Long-term Care Insurance services, you will be responsible for 10% of the service cost (20% or 30% if you have a certain level of income). In addition, if you use facility-based services, you will be responsible for food, room and daily living expenses. (Food expenses, room charges, and daily living expenses are determined by the contract for use, so they vary from company to company.)
About reduction system of user burden
1.High-cost nursing care service costs, etc.
When the monthly user burden (10%, 20% or 30% of the service cost) exceeds a certain upper limit, if you apply to the ward office, "expensive nursing care service costs, etc." will be refunded. For more information, please contact the Insurance and Pension Division of the ward office.
2.Reduce the burden of food and room charges (residential and stay expenses)
Food and room charges for entering the facility and using the short-term entrance (short stay) are usually borne by the user, but if you belong to a municipal tax-exempt household and meet certain criteria, apply to the ward office Insurance and Pension Division The out-of-pocket expenses are reduced according to your income.
Even if you are not eligible for the burden limit certification, if you are a household of two or more people and enter a nursing care insurance facility and pay for food and room, and meet certain criteria, apply to the ward office Insurance and Pension Division The self-pay is reduced (exceptional reduction measures).

3.High-priced medical care and high-cost nursing care combined system
If the total amount of each medical insurance (national health insurance, social insurance such as health insurance union, medical care system for the elderly) and The Long-term Care Insurance's co-payment for one year becomes high, the amount exceeding the specified co-payment limit will be refunded.
