Introduction of discounts for mentally ill persons at railway companies
It has been announced that JR and other railway companies will introduce discounts for mentally ill persons, but details have not yet been determined. We will inform you on the website as soon as it is decided, so please wait for a while.
Those who are eligible for personal organizers
Those with mental illness who have long-term restrictions on daily life or social life due to mental illness (*)
※Mental disorders include developmental disorders and epilepsy.
※You must be at least six months after your first visit (Shoshin) date.
※Intellectual disability is not subject to the Mental Handicapped Health and Welfare Handbook because there is a notebook of love.
About disability grade
Depending on the degree of disability, you will be certified from grade 1 to grade 3. (The first grade is the heaviest and the third grade is the lightest.)
Judgment of grade is made based on the contents of the submitted "Medical certificate for the Mental Handicapped Health and Welfare Handbook".
If you are receiving disability pension or special disability benefits due to mental illness, you can make a judgment by copying your certificate. In that case, the grade of disability pension is the grade of the Health and Welfare Handbook for the Mentally Handicapped. (For example, if disability pension is a second grade, the mental notebook is also a second grade.) ※Intellectual disability is not eligible.
About style of notebook
You can choose either paper style or card style. (There is no difference in the service available.)

※In general, paper styles tend to be issued earlier than card styles.
How to apply
There are two ways to apply: at your ward office (Elderly and Disabled Support Division) or by mail.

※It takes about two months from the application to the issuance of the notebook. If the medical certificate is incomplete, it may take some more time.
The period required for grant varies depending on the timing and content of the application. (Transfer outside the city, re-grant, and changes to the items described tend to be a little earlier.)
※You can pick up your notebook at the ward office Elderly and Disabled Support Division. (Even if the application is mailed, you can receive the notebook at the ward office.)
About validity period and renewal
◆The Health and Welfare Handbook for Mentally Handicapped is valid for two years.
◆You can apply for renewal at least three months before the expiration date. If you wish to continue using the notebook, be sure to apply for renewal.
(Example) In the case of the notebook above, the expiration date is March 31, 2022, written in (toshi). You can apply for renewal from January 1, 2022 because it is only three months ago. If you are using the services that can be received by presenting your notebook, we recommend that you apply immediately when it is time to renew.

※Years with February until the 29th are replaced with February 29th.
※The ward office and the paperwork center that accepts mail applications are closed on Saturdays, Sundays, holidays, and the year-end and New Year holidays, so applications cannot be accepted. Please apply as soon as possible.
◆Regarding the renewal application, Yokohama City will not notify you individually in advance, so please check the expiration date written in your notebook yourself.
About simultaneous application with independence support medical care (psychiatric outpatient medical care)
If you are also applying for the “Independence Support Medical Care (Mental Outpatient Care)” system that reduces medical expenses for outpatient visits, please refer to the following page for what you need to apply.

Inquiries about disability pension, etc. using My Number
If you are receiving disability pension or special disability benefits due to mental illness, you can submit a copy of the certificate to your personal organizer. In the past, Yokohama City had made inquiries to the pension Office, etc. on paper regarding the status of receipt of disability pension, etc., but from January 2024, we decided to use My Number to make inquiries.

※If the ICD code for the main mental disorder is F4 to F9, simply stating 11 (1) and (2) does not correspond to the "severe and continuous" of independence support medical care. ④Please confirm once again whether (6) or (7) is marked.
※If you would like to send a copy-type medical certificate, please call the Mental Health Consultation Center. (045-671-4455)
※It is not possible to answer the patient's application status over the phone. If you have received your application, you will be given a form called the reception confirmation slip (A4), so please check there. If you can call the paperwork center (045-671-3623), you can reply back.

For those who need a copy of the medical certificate in the application or trial of disability pension
If you need a copy of your medical certificate, please ask the hospital or clinics first.
If you are unable to receive a copy of your medical certificate at a hospital or clinics, you can receive a copy by requesting the City of Yokohama to disclose it.
※The reception desk for disclosure requests is "Ward Administration Promotion Division Public Relations Section in your ward" or "Citizen Information Center".
※It takes about one month from the request to the delivery of the copy.
※The method of grant is either "window (window of mental health consultation center)" or "mail (mailing fee paid by the person)".
※At the time of request, it is necessary to specify the documents to be disclosed, such as the "Medical certificate for the Mental Handicapped Health and Welfare Handbook submitted in 200", so please check with the submission address in advance when a medical certificate is required .
※Please note that Yokohama City will call the doctor who listed the medical certificate to confirm that the medical certificate may be disclosed.
※The preservation period of the medical certificate submitted to Yokohama City is 5 years. We can't handle anything older than that.
