Issuance of Mental Handicapped Health and Welfare Handbook

Introduction of discounts for mentally ill persons at railway companies

It has been announced that JR and other railway companies will introduce discounts for mentally ill persons, but details have not yet been determined. We will inform you on the website as soon as it is decided, so please wait for a while.

Those who are eligible for personal organizers

Those who have mental illness who have long-term restrictions on daily life or social life due to mental illness (*)
※Mental disorders include developmental disorders and epilepsy.
※The application must have passed at least 6 months from the date of first visit (Shoshin).
※Intellectual disability is not covered by the Mental Handicapped Health and Welfare Handbook because it has a notebook of love.

About disability class

Depending on the degree of disability, you will be certified from grade 1 to 3. (Grade 1 is the heaviest and the third grade is the lightest.)
Judgment of grade will be made based on the contents of the submitted “Medical Certificate for Mental Handicapped Health and Welfare Handbook”.
If you have a disability pension or a special disability benefit due to mental illness, you can also make a judgment with a copy of the certificate. In that case, disability pension's grade = the grade of the Mental Handicapped Health and Welfare Handbook. (For example, if disability pension is second grade, the mental handbook is also the second grade.) ※Intellectual disability is not eligible.

About the style of notebook

You can choose either paper or card style. (There is no difference in the services you can receive.)

※In general, the paper style tends to be issued earlier than the card format.

How to apply

There are two ways to apply at your ward office (Elderly and Disabled Support Division) or by mail.
Please confirm what you need for your application from the link below before applying.
●Those who apply at the ward office window → Please refer to (about application at each ward office window).
●Those who apply by mail → Please refer to the following link.
※It takes about 2 to 3 months from the time of application until the notebook is issued. The application period depends on the timing and content of the application. (Transfer outside the city, re-grant, and changes to the items tends to be a little earlier.)
※You can pick up your notebook at the ward office Elderly and Disabled Support Division. (Even if the application is mailed by mail, the receipt of the notebook is at the ward office.)

Date of validity and renewal

◆The Health and Welfare Handbook for Mentally Handicapped is valid for 2 years.
◆Renewal applications can be made three months before the expiration date. If you wish to continue using your personal organizer, be sure to apply for renewal.

Notebook

(Example) In the case of the notebook above, the expiration date is March 31, 2022 as described in (Details). You can apply for renewal three months before, so it is from January 1, 2022. If you are using the service that can be obtained by presenting your notebook, we recommend that you apply as soon as it is time to renew.

※The year in which February is until 29th is replaced with February 29.
※The ward office and the paperwork center that accepts mail applications cannot be accepted because they are closed on Saturdays, Sundays, national holidays, and the year-end and New Year holidays. I would like to apply as soon as possible.
◆Regarding renewal applications, Yokohama City will not give you individual notices in advance, so please check the expiration date written in your notebook.

About simultaneous application with independence support medical care (psychiatric outpatient medical care)

About inquiry such as disability pension using my number

If you have a disability pension or a special disability benefit due to mental illness, you can submit a copy of the certificate to be reviewed by submitting a copy of the certificate. Until now, Yokohama City has been making inquiries to the pension Office, etc. regarding the status of receipt of the disability pension, etc., but from January 2024, we have decided to make inquiries using My Number.

※If you are also using the independence support medical care (psychiatric outpatient medical care) system, be sure to fill out the columns 9-11 of the medical certificate. However, if you are hospitalized and are not expected to be discharged, you will not be able to use it as a medical certificate for independence support medical care.
※If you would like a copy-type medical certificate form, you will receive it at the ward office Elderly and Disabled Support Division window.

※⑨If it is not described in the items 11 to 11, it cannot be used for independence support medical care (psychiatric outpatient medical care). Please check with the patient to see if you have any plans to use it.
If the main mental illness ICD code for the main mental illness is F4 to F9, simply listing 11 (1) and (2) does not correspond to "severe and continuous" of independence support medical care. ④Please confirm once again whether (6) or (7) is marked with a mark.
※If you would like to receive a copy-type medical certificate, please call the Mental Health Consultation Center. (045-671-4455)
※It is not possible to respond to the patient's application status by telephone. If you have received the application, we will give you a form called a reception confirmation slip (A4), so please check there. If you call the paperwork center (045-671-3623) from the person, you can answer back.

For those who need a copy of the medical certificate in disability pension's application or trial.

If you need a copy of your medical certificate, please contact the hospital or clinics first.
If you cannot receive a copy of your medical certificate at a hospital or clinics, you can receive a copy by requesting a disclosure from Yokohama City.
※The reception desk for disclosure requests is "Ward Administration Promotion Division Public Relations Section in your ward" or "Citizen Information Center".
※It takes about one month from the time of the request to the issuance of a copy.
※The method of grant is either "window (window of mental health consultation center)" or "mail (paid by the mail fee)".
※At the time of request, it is necessary to specify a document to be disclosed, such as the "Medical Certificate for Health and Welfare Handbook for the Mentally Handicapped Health and Welfare Handbook submitted in FY2002", so please check with the submission destination in advance when the medical certificate is required .
※Please note that Yokohama City will call the doctor who has written the medical certificate to confirm that the medical certificate may be disclosed.
※The duration of the medical certificate submitted to Yokohama is 5 years. We can't handle anything older than that.
