You can only have one of them.
If you wish to change to a card, please return your current disability certificate to the city when handing the card. (Replace the card and paper at the counter.)

• In addition to making the face photo smaller, even if the photo is submitted in color, it will be printed in monochrome.
• For the certificate of the physically disabled, the name of the disability is listed on the front side.
• Regarding the certificate of the physically handicapped and the notebook of love, only the minimum required items will be posted. For example, the date of re-issuance of the certificate of the physically disabled, the previous grade, or the past judgment record of the love notebook are not described.
• For the Mental Handicapped Health and Welfare Handbook, the expiration date after the renewal is written on the back.
