You can only have one of them.
If you wish to change to a card, please return the paper-style notebook you have to the city when you hand the card. (Replace card style and paper style.)

- Since it is made of plastic, it has excellent durability, and since the size is small, it is easy to put in a wallet etc. (There is no cover.)
- In addition to the smaller face photo, even if a photo is submitted in color, it will be printed in black and white.
- The validity period after renewal is indicated on the back.

- You can give a cover to put your notebook.

- There is no difference in welfare services that can use both paper and card styles.
※If you currently have a paper-style notebook and do not wish to change to a card style, you can use your current notebook as it is.
In order to prevent the spread of COVID-19, mail will be sent by registered mail for the time being.
(For those who apply during the pre-registration period from January 15 to the end of March 2021, we will send it to the applicant Address who was filled out at the time of application from the second week of June 2021.)
The card issuance fee is free, but it is necessary to prepare a postage stamp when applying for a change to the card style.

If it is difficult to print, please send a 94 yen stamp to the paperwork center, stating that you would like to send an application form for changing the card style disability certificate and your name and destination Address. We will send you the application documents as soon as it is confirmed at the Administrative Processing Center.
