Can I have both card style and paper forms?
A

You can only have one of them.
If you wish to change to a card, please return your current paper-style notebook to the city when you hand the card. (Change card style and paper style.)

What is the difference between a card style and a paper style?
A

[Card]

  • Because it is made of plastic, it has excellent durability, and the size is small, making it easier to put it in wallets. (There is no cover.)
  • In addition to making the face photo smaller, even if the photo is submitted in color, it will be printed in monochrome.
  • The validity period after the renewal is indicated on the back side.

[Paper]

  • You can hand the cover to put your notebook.

[Common]

  • There is no difference in the welfare service that can be used in both paper and card forms.

※If you currently have a paper-style notebook and do not wish to change to the card style, you can use your current notebook as it is.

How can I get a card-style notebook?

To prevent the spread of new coronavirus infection, mail will be sent by registered mail for the time being.
(For those who applied during the pre-registration period from January 15 to the end of March 2021, we will send it to the Address of the applicant who filled out at the time of application from the second week of June 2021.)

Is there a fee?
A

The card issuance fee is free, but you will need to prepare a postage stamp when applying for a change to the card form.

Where can I get the application form?
A

You can download it from the following.
If printing is difficult, please send a 94 yen stamp to the paperwork center, stating that you would like to send a card form for changing your personal disability certificate, and your name and destination Address. We will send you the application form as soon as it is confirmed at the paperwork center.

Can I apply on my behalf?
A

It is possible.
Enter the name, Address, etc. of the person applying for proxy in the applicant column.
Information on how to receive it will be sent to the applicant.
