Issuance of love notebook (nursing notebook)

Those who are judged to have intellectual disability at the children's guidance office or the Rehabilitation Consultation Center for Persons with Disabilities

Depending on the degree of disability, it is certified as A1 (most severe), A2 (severe), B1 (medium), and B2 (mild).
The content of the support may vary depending on the degree.

• Most severe (A1): 20 or less
• Severe (A2): 21 to 35 or less
• Medium (B1): 36 to 50 or less
• Mild (B2): 51 to 75 or less

※If you have an autism spectrum disorder, etc., and the judging body determines that you have intellectual disability, you will be issued a notebook of love.
(If you do not have intellectual disability, you will not be eligible for a notebook of love.)

１.　A photo of the upper body taken recently (within 1 year). 4cm long x 3cm wide.
２.　Wearing hats, sunglasses, masks, and Polaroid photos are not allowed.
３.　Photos with no background as much as possible, such as ID photos. It doesn't show any other person.

After the application, each consultation office will inform you of the date and time of the judgment.

• Under 18 years old: Each city children's guidance office
• Age 18 and over: Rehabilitation Counseling Center for Persons with Disabilities

It takes about 2 to 2.5 months after application. (It may take more time depending on the status of the judgment.)

In Yokohama City, we introduced the card style of the Handbook for Persons with Disabilities in June 2021. At the time of application, you can choose either paper style or card style.
If you already have a notebook and do not wish to issue a card style, you can use your current paper-style notebook as it is.

If you already have a notebook of love in Yokohama City, we are accepting mail only for applications for re-issuance of "lost notebook" or "when notebook is damaged".

・A letter informing you that your personal organizer has been completed will be sent to Address.
・The place where the completed notebook is issued is Health and Welfare Center of each ward office.
