Mental Handicapped Health and Welfare Handbook Card Style Change Application

With the enforcement of the Ministerial Ordinance (Ministry of Health, Labour and Welfare Ordinance No. 48 of 2019) amending a part of the "Regulations for Enforcement of the Law Concerning Mental Health and Welfare for the Mentally Handicapped" on April 1, 2019, the Handbook for Persons with Disabilities is now available in card format. Yokohama City accepts advance applications for those who have a certificate of disability living in Yokohama City and are accepting advance applications for those who wish to have a certificate of disability in the form of card format for handover from June 2021. In addition, those who continue to use the current paper-style disability certificate do not need to complete the procedure.

  • It becomes a plastic card type notebook with excellent durability and portability.
  • It has embossed processing and cuts, and can be distinguished from other cards.
  • Even if you submit the photo in color, the face photo will be all monochrome and the size will be smaller.
  • The renewal expiration date will be indicated on the back side.
  • There is no difference in the various services available between a paper-style disability certificate and a card-style disability certificate.
  • Toll road toll discount registration, etc. will be described in the remarks column on the back. (planned)

Those who can apply

Those who currently live in Yokohama City and have a Mental Handicapped Health and Welfare Handbook
If you need to renew by the end of May 2021, please apply after receiving your personal organizer after the renewal procedure.
※By the end of May 2021, those who receive a notebook application / grant by new, out-of-city transfer, and re-grant application will be paper-style notebooks.

How to apply

Please submit the following three items by mail.
Please prepare your own envelope for mailing the downloaded application form. (A commercially available envelopes are also acceptable.)

4cm long, 3cm wide, hat off, facing the front
※Please write your name, date of birth, and ward name on the back of the photo. ※Card style Please paste it in the designated position on the card style disability certificate change application form.

③Copy of the issued Mental Handicapped Health and Welfare Handbook Please copy the face with your face photo.

About handing over

For the time being, mail will be sent by registered mail to prevent the spread of COVID-19 infection.
(For those who applied during the pre-registration period from January 15 to the end of March 2021, we will send it to the applicant's Address listed at the time of application from the second week of June 2021.)
