Mental Handicapped Health and Welfare Handbook Card Style Change Application

As of April 1, 2019, the Ministerial Ordinance (Ministry of Health, Labour and Welfare Ordinance No. 48 of 2019) amending a part of the Enforcement Regulations on Mental Health and Welfare for Mentally Handicapped Persons became a card format. For delivery from June 2021, Yokohama City is accepting advance applications for those who have a certificate of disability living in Yokohama City and wish to have a certificate of disability in a card style. In addition, procedure is unnecessary for person having you continue using current paper-style disability certificate.

Mental Handicapped Health and Welfare Handbook


  • It is a plastic card-shaped notebook with excellent durability and portability.
  • There are embossed and cutouts, which can be distinguished from other cards.
  • Even if you submit a photo in color, the face photo will all be monochrome and the size will be smaller.
  • The updated expiration date will be indicated on the back.
  • There is no difference in the various services that can be received between the paper-style disability certificate and the card-style disability certificate.
  • Toll road toll discount registration, etc. will be described in the remarks column on the back. (planned)

Those who can apply

Currently, those who live in Yokohama City and have a Mental Handicapped Health and Welfare Handbook
If you need to renew by the end of May 2021, please apply after receiving the notebook after the renewal procedure.
※By the end of May 2021, those who receive the application / grant of the notebook by new, out-of-city transfer, or re-grant application will be a paper-style notebook.

How to apply

Please submit the following three items by mail.
Please prepare your own envelope for mailing the downloaded application form. (A commercially available envelope is also acceptable.)

4cm long, 3cm wide, hat off, facing front
※Please write your name, date of birth, and ward name on the back of the photo. ※Please paste it in the designated position of the card style disability certificate change application form.

③Copy of the issued Mental Handicapped Health and Welfare Handbook Please copy the face with your face photo.

About handing over

In order to prevent the spread of COVID-19, mail will be sent by registered mail for the time being.
(For those who apply during the pre-registration period from January 15 to the end of March 2021, we will send it to the applicant Address listed at the time of application from the second week of June 2021.)
