It is guidance when we apply for "Mental Handicapped Health and Welfare Handbook" at "ward office window".
Check what you need for the application and complete the procedure at the Elderly and Disabled Support Division window.
(If you live in a group home for the disabled or a nursing home, the ward office to apply may be different from usual.)

Type of application

1.New application

This is required documents for the first time to apply for the Mental Handicapped Health and Welfare Handbook.
When you apply for independence support medical care (psychiatric outpatient medical care) at the same time, please move to (simultaneous application at ward office) (simultaneous application by mail).

Explanation of simultaneous application


Judgment of the Mental Handicapped Health and Welfare Handbook is based on a medical certificate, but if you are receiving a disability pension or a special disability benefit due to mental illness, submit a copy of the pension certificate etc. You can apply without submitting a medical certificate). In that case, the grade of disability pension will be the grade of notebook.

When applying for a pension certificate

2.Letter of Consent Form (PDF: 116KB) (This is a written consent form to confirm receipt status from Yokohama City to pension Office, etc.)
3.seal (shachihata)

When applying with a medical certificate

◆Please print it out here and give it to family doctor. Depending on the hospital, you may have a medical certificate form for Yokohama City, so please check once.

◆Please choose either A3 / single version or A4 / 2-size version depending on the printer status. (A3 / single version should be enlarged to A3 and print. When printing in A4 ・ Two-page versions, print on two sides instead of double-sided printing. )

◆Please check if 6 months have passed since the date of first visit (Shoshin).

◆This time, I will not apply for the independence support medical system, but it may be done later.…In that case, please write the items 9-11 in the medical certificate.

When applying for a pension certificate

◆The so-called "disability pension" is targeted. Mental disorders include developmental disorders and epilepsy, but exclude intellectual disabilities. (Because intellectual disability is not subject to the Mental Handbook.)
◆Old-age pensions and survivors' pensions are not eligible.
◆From January 2024, we will refer to the pension office with my number.

③My Number Card

◆If you do not have a My Number card, you can also use a number notification card + identity verification document. If you have any of them, please let us know at the counter.

④Identity verification documents

◆③This is not required if you have a My Number card.
◆③If you do not have a My Number card, you will need a personal identification document with a photo of your face, such as a license or passport. If you do not have a document with a photo, please bring two health insurance card, pension notebook, and medical care certificate for independence support.

⑤One face photo (4cm long x 3cm wide)

◆Please write your name and date of birth on the back of the photo.
◆There are two types of notebooks: paper style and card style, and you can choose either one.
・Paper style → It is okay if you can bring the photo when you finally issue your notebook at the counter. Of course, you can submit it at the time of application.

2.Renewal Application and Reapproval

If you already have a personal organizer, you can apply for renewal within the validity period (= renewal application) or if you have a notebook but have expired, you will apply again (= reapproved) required documents.
When you apply for independence support medical care (psychiatric outpatient medical care) at the same time, please move to (simultaneous application at ward office) (simultaneous application by mail).

◆Renewal applications can be made three months before the expiration date.

In the above example, the expiration date is March 31, 2022. In that case, you can apply for renewal from January 1, 2022, three months ago, you can apply for renewal.

※For February, the year until 29th is replaced with February 29.
※The ward office cannot accept applications on Saturdays, Sundays, holidays, and the year-end and New Year holidays, so they cannot accept applications. Please apply as soon as possible.
◆Yokohama City has not sent any notice of the renewal date. After confirming the expiration date written in your notebook, please apply for renewal so that you do not forget.
◆If you apply for renewal by the expiration date, the period will not be interrupted, but it will take about 2 to 3 months from the time of application to the issuance of the notebook. (It depends on the time and content of the application.) If you show your notebook and use another service, you may not be able to receive the service because you do not have your notebook at hand, so if it is 3 months ago We recommend applying immediately.

3.Grade change application

This is the required documents if you want to change the grade of your notebook.

◆The grades of the Mental Handicapped Health and Welfare Handbook are three levels: 1st, 2nd and 3rd grade. (Grade 1 is the heaviest and the third grade is the lightest.)
◆If you wish to change your grade, you will need to submit a copy of your medical certificate or pension certificate and have to be reviewed again.
◆If the grade change is approved, the validity period will change from the notebook you have now.
◆If the grade change is not approved (= grade has not changed), a notice of disapproval will be sent.

4.Out-of-city transfer application

This is required documents when you move to Yokohama from a municipality other than Yokohama.

If the previous municipality has issued a Mental Handicapped Health and Welfare Handbook, the contents will be taken over until the expiration date of the notebook.
If less than three months have passed until the expiration date and you have not completed the renewal procedure at the previous municipality, you need to apply for a renewal in Yokohama City.

5.Application for change of information

This is required documents when the name and address (moving in Yokohama) has changed.

If you move from Yokohama to a municipality other than Yokohama, you will need to apply for a suburban transfer at a new municipality. In that case, please be careful not to lose it because it is necessary to bring the notebook issued in Yokohama City.

7.Card style change application

This is the required documents if you have a paper-style notebook and want to change it to a card style.
※If you want to change your card-style notebook to a paper style, you must apply for a reissue.

8.Notification of return

This is the required documents when the notebook is no longer needed.

Returning your personal organizer may have an effect in various situations. Please read and approve the guidance (PDF: 221KB) before returning. Once returned, it cannot be resurrected. If you need a personal organizer again, you will need to apply for a new one. (The number of the notebook will also be changed.)

9.About the flow from application to notebook issuance

◆It takes about 2 to 3 months from application to the issuance of the notebook. It depends on the timing and content of the application.
◆As a result of examination, when grant of notebook is decided, we send guidance called "news of grant of mental patient health welfare notebook" from ward office. Please bring your belongings written there and receive a notebook at the ward office Elderly and Disabled Support Division window. (Only card style change, we mail notebook directly from health consultation center of city heart.)
◆As a result of examination, when it becomes disapproval, we send notice of disapproval from health consultation center of city heart.

It has been a little since I was diagnosed with a mental illness. Can I apply for a notebook?
A

The notebook will not be approved unless more than 6 months have passed since the date of first visit (Shoshin). Wait for 6 months to apply.
※If you are transferred, "first visit (Shoshin)" does not mean the date of first visit (Shoshin) at the hospital you are currently attending, but the date on which the disability was first diagnosed.

I am currently in the hospital, can I apply for a notebook?
A

You can apply even if you are in the hospital. However, the independence support medical care (psychiatric outpatient medical care) system, which can be applied at the same time with the same medical certificate, is a system for outpatient visits, so if you are still hospitalized at the time of examination and there is no prospect of discharge, independence support medical care will not be certified Become.

Is intellectual disability covered by the Health and Welfare Handbook for the Mentally Handicapped?

Intellectual disability is not covered by the Mental Handicapped Health and Welfare Handbook because there is a separate system called the "Love Notebook".

A

medical expenses, such as Psychiatry, will be cheaper for independence support (psychiatric outpatient care) system.
Just applying for a notebook does not mean that you have applied for independence support medical care, but a separate application is required.

Are there any other requirements for the face photo besides size?
A

Photos must meet the following conditions.
◎4cm long x 3㎝
◎Do not wear a hat (it is OK to cover the head with a cloth as far as the contour of the face is visible for religious or medical reasons)
◎I don't wear sunglasses
◎The upper body is shown and can be distinguished from the person.
◎There is no other person in the picture
◎Photographs taken within one year
◎When printing at home, etc., on the photo mount (copy paper, it may break)
◎Polaroid photo, Cheki NG
※In the case of the card style, the finish will be black and white even if you submit a color photo.

A

Notebooks can be issued only at the ward office Elderly and Disabled Support Division window. If you are unable to go to get it, you can also pick it up by a proxy. (Identification documents for the agent are required.)
※Only the change from paper style to card style will be mailed from the Health Consultation Center in the city.
