• Letter of Pass, Certificate of Pass, or Certificate of Pass (both original)
• A certificate of all matters of family register (certified copy), family register personal matters certificate (abstract), copy of resident certificate (with your permanent address (for foreign nationals) and My Number is omitted) (issued within 6 months, not copyable)
  ※For foreign nationals listed in each item of Article 19-3 of the Immigration Control and Refugee Recognition Act, a copy of your passport or other documents certifying your identity (please bring the original).　
• Doctor's certificate (issued within one month)
