• Fugu knives license renewal (reissue) application form

  It's at the Health and Welfare Center window.

• Fugu knives license (in case of change or damage to items described)
• 2 photos (front facing, hat, upper body image 4.5cm long, 3.5cm wide)
• Fee 2,710 yen (Payment notice will be given at the counter at the time of application)

  However, if you rewriting and re-grant at the same time, it will be 5,420 yen.

• In the case of name change

  Certificate of entry in certificate of partial family register or family register (both issued within six months after issuance)
