Notification of when food-related business is stopped (closed)

If you are applying for a business license or a business report for a store that goes out of business using a food hygiene application system, please use this system to complete the closing procedure.
If you have a business license at hand, please discard it immediately after notification of acceptance completion.

Please submit a report using one of the following methods.

Please bring one copy of the following documents at a time.

• Notification of business closure (Excel: 23KB)

  Please fill in the form with reference to the entry example (PDF: 420KB).
  It is also available at the counter of Nishi Ward Health and Welfare Center Health Sanitation Division.

• Operating permit (only for business licensed facilities)

Please mail one copy of the following documents by special record mail.

Please file from Yokohama-shi electronic application, report service (outside site).
To use this service, you need to obtain a user ID.
Enter the "Procedure List (for businesses)" at the top of the top screen, and search for "Notification of closure of food-related business" by keyword search.
If the business license facility is closed, please attach an image of the business license.

After confirming that there are no defects in the contents of the closing notice, we will send you a notice of completion of acceptance.
If you have a business license at hand, please discard it immediately after notification of acceptance completion.
