Survey on medical care for severely handicapped children and persons

In Yokohama-shi, we grasp living situation of severely handicapped child, person living in the city and family being cared for, medical institution going to hospital and the use situation of welfare service in 2016, We conducted a questionnaire survey to help promote future measures.
We will announce the statistical processing results.

A questionnaire has been sent to those who fall under all of the following 1 to 3.
As of 10:00 a.m. on December 21, 2016
1 Those who have resident registration in Yokohama City
2 Person who possesses certificate of the physically disabled (limb disability) first grade or second grade
3 Those who have a love notebook (nursing notebook) A1 or A2
