If a child registered in Kohoku Ward is enrolled in a private or national elementary or junior high school, it is necessary to submit a "Notice of Study Abroad" to the Kohoku Ward Office.
What you need to prepare
“Notice of Out-of-area Study”
Download the "Notice of Out-of-Area Study (PDF: 50KB)" and list all required items.
(For Phone number, please fill in the number that can be contacted during the day. If you have any questions, we may call you if you have any questions.)
Letter of consent for attending school (enrollment) (permit)
The original document (copy not allowed) that indicates that the school you plan to attend, such as a “School (entry) consent form (permit)” (permitted).
Please note that the “Success Notice” is not a document that has been approved for attending school, so it cannot be used in the “Notice of Out-of-area Study”.
