I lost my welfare special ticket (welfare pass), what should I do?

In the following cases, you will be re-issued at the ward office of your ward.

(1) In case of loss
 We will reissue it (limited to once during the validity period) when it is deemed that the welfare pass has been lost due to intentional or serious negligence.
 ※Please report the lost item to the nearest "Koban" (police box) or police department.
(2) In case of theft
 Please notify the nearest "Koban" (police box) or police department. It will be reissued after confirming the facts by documents or inquiry.
(3) In the case of fire, storm and flood damage
 It will be re-issued after confirming the facts with documents that can confirm the facts.
(4) In the case of damage or damage
 If use becomes difficult, a new ticket will be reissued in exchange for a welfare pass. For more information, please contact the ward office of your ward.
