2.Procedures for designated renewal

3.Procedures for Changes, etc.

If there is a change in the name and location of the operating corporation, such as the designated welfare service establishment, or the representative of the corporation, manager, service manager, etc., the change registration form must be submitted within 10 days from the date of the change. 7.Procedures for Information Disclosure System
