1.Outline of group homes for persons with disabilities

 Group homes for persons with disabilities (communal living assistance) are one of the disability welfare services prescribed in Article 5, Paragraph 1 of the Act (Comprehensive Support for Persons with Disabilities Act) for comprehensively supporting the daily and social life of persons with disabilities. In one of them, we provide consultation, bathing, excretion or meal care, and other daily life assistance in cohabiting dwellings. In addition, it is expected to be a recipient of regional transition from entrance facilities for persons with disabilities under the Comprehensive Support for Persons with Disabilities Act.
 In Yokohama City, we will continue to promote the development of group homes for persons with disabilities so that people without disabilities can live in the community with peace of mind.
※From April 2014, cohabitation care (care home) has been unified into cohabitation support (group home).

2.About new establishment assistance of group home for the disabled ※Application for new establishment in fiscal 2024 has ended.

If you wish to subsidize the cost of new construction, we will recruit once a year, so please apply.
Usually, from the end of August to the beginning of September, information on the new establishment of the next fiscal year will be posted on the website.

3.On-the-job guidance

 We provide on-the-job guidance once every three years. About result notice, we notify by document after examining necessity to notify as city based on matter, hearing and comment that we confirmed locally. If you need an improvement report, use the following form to prepare and submit a report using the following form.
 When it is not recognized as appropriate business implementation, it may become a designated cancellation as a group home for the disabled or a part or all of the subsidy.
 By the month before the month when the on-the-job guidance is implemented, we will contact you and notify us of the implementation.

・About submission of "pre-submission materials"
 Pre-submission materials should be submitted using the following Yokohama City Electronic Application and Notification System on the designated date (approximately 14 to 10 days before the implementation date) where (notification) is described. Please submit.

・About submission of "improvement report"
 The results of the on-the-job guidance will be notified at a later date by the on-the-job guidance result notice. 5.About accident report

If an accident or incident occurs while the service is being provided (including during tsusho or returning home for tsusho offices), the first report will be made to Kanagawa Prefecture and Yokohama City by telephone, and then promptly report "Accident Report" Must be submitted.

6.Billing

(1) About billing error
①Please fill in required items on the "Request Error Receipt Form (Excel: 24KB)" and send it to kf-syohome@city.yokohama.jp.
 The subject must be as follows.

②Please set a password as personal information is included in the description.

②Deadlines
Until 23:59 the day before the last open agency of the month
※Example: In the case of March 2022, the deadline is 23:59 on March 30.
※If it coincides with Saturdays, Sundays and holidays, the deadline will be moved forward.
※It is from December 29 that Yokohama City will be closed at the end of the year.

(3) About the case of requesting independence support payment costs directly to Yokohama City
Because the system cannot charge the National Health Insurance Federation for the independence support benefit expenses for community living assistance (group home), please mail the following required documents.

(b) Documents that show the rent burden of the user (important matter description, etc.)
(c) Copy of recipient ID

8.About rent subsidy addition when a user whose Yokohama City is the implementing agency of support enters a group home outside Yokohama City

If the user determined by Yokohama City uses a group home outside Yokohama City, you may be able to request a rent subsidy addition depending on the standards of the local government where the group home is located. For details, please refer to "Yokohama suburban person with a disability group home alone addition supply summary" (PDF: 362KB).
In addition, please submit the first style of the above summary by the month before the month to request in principle.
If you do not submit it, we may reject it, so please do not forget it.
(1) Additional operating expenses
 ① Base amount
   Based on the standards of the local government (*) that has jurisdiction over group homes for persons with disabilities.
 ② Calculation method
   Based on the standards of the local government (*) that has jurisdiction over group homes for persons with disabilities.
 ③ Targets
   According to the standards of the local government (*) that has jurisdiction over group homes for persons with disabilities outside the city, out of the additions that are not eligible.

※Local governments are limited to local governments in Tokyo, Kawasaki City, Sagamihara City or Kanagawa Prefecture.

(2) Rent subsidy addition
 ① Base amount
   Based on the standards of the local government that has jurisdiction over group homes for persons with disabilities.
 ② Calculation method
   Based on the standards of the local government that has jurisdiction over group homes for persons with disabilities.
 ③ Targets
   According to the standards of the municipality that has jurisdiction over group homes for persons with disabilities outside the city, the implementing agency for assistance is not the local government.
