Regarding the report of the test results, due to the rapid increase in the number of tests, the results are usually announced about 3 days after the arrival of the sample at the test site, but it is currently taking more time. As soon as the sample arrives at the laboratory, the test will be performed sequentially, so we apologize for any inconvenience, but please forgive us.

About PCR test targeting at worker such as facilities for the disabled in the city that we carried out from Monday, May 31, 2021 to Tuesday, December 14, 2021, March 15 It will be extended until Tuesday, December 15. Applications can be made up to once each business operator. From January, some business flows will be changed as follows. We ask for your continued active consultation.

(1) Collection of Sample Kits
・The test trustee must submit (shipped) samples in Yu-Pack instead of heading for collection.
Be sure to check the manual and "Frequently Asked Questions" as the series of flows from the arrival of the kit will change.
・When submitting samples, please enter required items in the “Sample ID / Name Sending Form” sent separately by the contractor and reply to the following e-mail:

Q1. If you are based in Yokohama and have a branch in Kawasaki, can you apply up to the staff of the branch?
A1. You cannot apply for a branch office. Please apply for each office where you are located.
Q2. The designation of business establishments is received in other cities, but the business establishments (facilities) are located in Yokohama. Are you eligible for this project?
A2. Eligible. If you would like to apply, please apply for each office.
In addition, when you have you apply, you may have you push forward registration with section column in charge of designated right holder (prefecture municipalities) leaving blank. (Because it is not a mandatory item, it does not result in an error.)
Q3. There are multiple offices within the corporation, can I apply at once?
A3. You will not be able to apply at once. Please apply for each office.
Q4. Are part-time workers who work about once a week also subject to inspection?
A4. You may be eligible. The type of employment (full-time employees, contract employees, part-time workers, etc.) does not matter, so please target staff who are deemed necessary as facilities and offices.
Q5. Is it acceptable for employees who are also working in other industries, such as nursing care services for the elderly?
A5. You may be eligible. In addition, about the staff who concurrently serve multiple facilities with the same service, there is no overlap.

Q6. Is there an upper limit on the number of applicants?
A6. I don't have anything special.
≪ About application form ≫
Q7. When applying for a web form in Kanagawa Prefecture, my own Name of the facility will not appear even if I search for facilities.
A7. In the Kanagawa web form search, even the same word does not correspond to notation fluctuations such as hiragana, katakana, kanji, alphabet uppercase letters, lowercase letters, full-width or half-width. Therefore, it is necessary to enter the correct facility name as a search word. If you still can't find it, press "Click here" at the bottom of the facility name new registration search screen to register.

Q8. At the time of new registration, what should I do if the type I am doing in the selection column of business type is not listed in the pull-down list?
A8. I'm very sorry, but you can only select the type you are currently viewing. We are planning to add the type in the future, so please select the type that you think is most appropriate at this time. (Change of type can be changed from a later date.)
Q9. If multiple designations are made at one business establishment, what should I select for the "Service Classification" on the facility basic information screen?
A9. Please select the type with the largest number of people (capacity) or the type with central functions at the facility or office.
Q10. Should I enter the total number of people per day or per week?
A10. Enter the number of people per day.
Q11. How should I enter the total number of people if multiple designations are made at one business establishment?
A11. Enter the total number of businesses that have been specified.
Q12. How should I fill in the total number of services without capacity?
A12. Please consider the current number of subscribers as capacity and enter them.
Q13. How should I count the number of staff if one staff member also serves multiple jobs (managers and other jobs (supporters, consultation support specialists, etc.))?
A13. Please include the number of people in all positions.

Q14. Where should the section in charge of the designated right holder (prefecture municipalities) enter?
A14. Please enter each business jurisdiction section of disability welfare health department or child welfare health department.
If the competent department corresponds to more than one section, enter the name of the section that has jurisdiction over the type that has a central function at the facility / office.
Q15. If you have any questions about Cooperating Medical Institutions, should you check the situation with Cooperating Medical Institutions?
A15. You can respond to the extent that you know at the business site.
Q16. Regarding questions from the PCR testing organization in Cooperating Medical Institutions, do you mean that the sample collected this time will be tested at the institution that answered the question?
A16. PCR tests for this project will be conducted by an inspection company contracted by the prefecture and Yokohama City.
Q17. How long does it take from the new registration of the facility name to the entry of the inspection application?
A17. Registration will be completed in about one business day. When you receive a notification by e-mail as soon as it is completed, please apply again from the WEB form.
Q18. Facility name A new registration has been made, but what happens if there is no notification of registration completion by 16:00 on Tuesday, the end date and time of each application?
A18. Since the application has not been completed by 16:00 on Tuesday, the application will be accepted from 9:00 on the next Saturday. Therefore, please proceed with the procedure with plenty of time.
Q19. When I searched the facilities, I found Name of the facility, but Address was old before the relocation, so I registered a request for correction. How long will it take to enter an inspection application?
A19. In the case of correction request registration, it is the same as for new registration. Please check A17 and A18. If Address is incorrect, it may interfere with sending test kits or collecting samples, so be sure to correct information before applying.
Q20. I noticed an error in Address after applying, so I would like to fix it in a hurry. What should I do?
A20. First of all, please contact the Health and Safety Division by phone. After that, please request the correction of Address from the application form (press <Click here> at the bottom of the Name of the facility search column of "Facility Basic Information").
Q21. If you want to increase the number of inspectors after applying, is it possible to respond?
A21. It is not possible to add the number of tests after applying. Please carefully consider the subject of the test before applying.
≪ Sample collection ≫
Q22. Does the facility have to ship the sample submission? Is it possible to have them come to the collection as before?
A22. From January, the testing company will not go directly to the facility to collect samples. Please be sure to return it in Yu-Pack by the specified method.
Q23. After applying, I received an email from an address different from Yokohama City, what should I do?
A23. You will receive an e-mail from the inspection company each time you send the kit with your application. Each e-mail is accompanied by an Excel file with the ID number assigned to the sent kit, so be sure to keep it. Also, the password will be changed every time, so be sure to keep it until you receive the test result report.
Q24. I received the kit, but it contains a different package than the conventional one, and how should I collect the sample?
A24. Please check the included materials and the procedure and precautions for submitting samples (to the inspection trustee's website) (outside site) and respond accordingly.
Q25. When collecting samples, I spilled the conservative fluid, what should I do?
A25. If spilled on the floor, wear rubber gloves to prevent touching with bare hands and wipe it off. If it adheres to the skin, wash it off with a large amount of water as described in the manual.
Q26. Since the samples of all the subjects did not collect during the sample return period, can I send the kit after all the samples were collected?
A26. Be sure to return (shipped) during the return period and within 4 days from the collection of the first bonito sample.
Q27. If an unused kit occurs, can I keep that kit for the next application?
A27. Each kit has its own unique ID number and is managed for each test, so it cannot be used for the next time. If an unused kit occurs, please return it when sending the sample.
Q28. If only the cushioning material used when returning the kit is included, the secondary container will not be fixed and you will not mind, so can you add something at the facility side?
A28. After storing the secondary container, if you do not mind with the included cushioning material alone, you can put your newspaper or flyer.
Q29. When do you know the test results?
A29. You will receive the results by email approximately 1-3 days after the returned sample arrives at the testing company. In addition, unlike until December, there is no fax transmission if a positive is found.
Q30. The test result arrived by e-mail, but the attached file has a password and cannot be opened.
A30. When applying for the relevant inspection, we will inform you of the kit and provide you with a password at the same time, so please check past e-mails.
≪ Others ≫
Q31. Do all staff working at the office have to be inspected?
A31. The test itself is optional, so I hope that you can apply after respecting the staff's intention to consult.
Q32. Can I get a certificate of test results?
A32. This time, we will not be able to issue certificates due to screening tests.
Q33. Is there anything to keep in mind about the test results?
A33.PCR tests may be false negative (positive but negative), so please do not rely on test results and take thorough infection control measures on a daily basis.
Q34. How do you respond when a positive person comes out?
A34. First of all, please stop working for those who have been confirmed positive. In addition, please inform the public health center at the location of the office that a positive person has been confirmed.
