About new business designation application

Please confirm "disability welfare information service Kanagawa (outside site)" for procedure and style about designated application.
The flow up to the designation is as follows.
1.Contact the section about two months before the designated month to make an appointment.

2.Preparation of application documents
 ※Please submit it to this section by mail, etc. at least one week before the interview date.
3.Interviews by the beginning of the month prior to the designated month
 ※During the state of emergency, we will send you an email to prevent the spread of infection.
 ※Please note that we may have an interview before preparing the application documents.
4.Submit application documents by the 15th of the previous month of the designated month

About false claims

When correction is necessary for request contents such as person with a disability independence support payment costs, we perform error withdrawal request from Yokohama-shi electronic application, report system (new) (outside site).
[Deadline for malpractice]
Until 23:59 the day before the last opening of the month
Example: In the case of April 2022, it will be until 23:59 on April 27.
※If it overlaps with Saturdays, Sundays and holidays, the deadline will be brought forward.
※Yokohama City will be closed at the end of the year from December 29.

・As for the user information registration method at the time of the first application, please see Yokohama-shi electronic application, report system operation manual (outside site).
・If you need to reclaim, please reclaim to the National Health Insurance Federation after the error is withdrawn.
・The ID and password registered in the old system have been reset.
 When applying for the first time with the new system, please register again.
・If the number of claims exceeds 30 for one service, we will provide an Excel format separately.

※At the time of presentation to ward independence support meeting, please confirm requirements in self-check sheet before presentation, and please submit in conjunction with report and record book.
