Provision of information on remuneration revision in FY2021

(For Designated Disability Welfare Service Offices) ※"Revision of Operational Regulations in conjunction with Further Promotion of Prevention of Abuse of Persons with Disabilities and Proper Restriction of Physical Restraints" has been posted (January 28, 2022).※

Regarding further promotion of prevention of abuse of persons with disabilities and the revision of operating regulations accompanying the mandatory appropriateness of physical restraint

From April 2022, efforts to further promote the prevention of abuse of persons with disabilities and to optimize physical restraints will become mandatory. Please revise the operating rules and submit a notification of change. March 25, 2022 (Fri.) ※Must arrive※

1 Revision of the Operating Regulations  
In the operating rules of each business establishment, it is necessary to stipulate "items related to the establishment of an abuse prevention committee" as "matters related to measures to prevent abuse". Please revise the Operating Regulations by March 2022.

2 Submission of a change notification: If you change the operating rules, you will need to submit a “Notification of Change”. 
 (1) Notification method    
  There are the following two notification methods for notification of changes in the operating rules concerning measures to prevent abuse.    
  Please submit a report by either online application or a paper report.
          
  A (recommended) Online application using Yokohama City Electronic Application and Notification System        
    Notifications can be made by attaching electronic data from the following URL. There is no postage charge.

   Report on paper B        
    As before, notifications by paper (postal mail) are also accepted. 
(2) Deadline for notification      
   [March 25, 2022 (Fri.) * Must arrive * Please submit a report.    
  ※If there are any changes other than the operating rules, please submit a report on paper (mail) as before in conjunction with other reasons.

【Q&A】
Oh Do you have both online and paper?
⇒In the case of only changes to the operating rules, either one of them is acceptable.
 However, if there is a change other than the operating rules, it will be submitted on paper together with other reasons for the change.

Oh Is seal unnecessary for notification?
⇒No seal is required for notification of change (Form 5).

Oh In the online application, do you attach the form of the change notification form?
⇒What you have entered in the online application form will be output as a notification of change (No. 5 style).
 It is not necessary to create a separate notification of change (No. 5 style).
 After you enter required items on the form, please upload and submit the modified operating rules.
